Discharge Planning Medication Encourage client to determine and know the prescribed medications, including the dose, the

desired amount and the possible adverse effects of the prescribed medications. Provide necessary and important informations about the medication as to understand the relevant informations regarding the therapeutic regimen to promote wellness. Encourage client to report unusualities observed when taking the medications such as hypersensitivity reaction to prevent the existence of an occuring problem. Exercise Encourage client to exercise regularly. Treatment Encourage the client to understand the importance of reporting to the phycician promptly unusualities felt or observed. Instruct the client to take the medications on time and with the right dose to prevent complications. Health Teaching Teach the patient sign and symptoms of hypoglycemia and hyperglycemia Teach the patients about medication purpose, dosage, route, and possible side effects of all prescribed medications. In patients with self-administer insulin, demonstrate patient the appropriate preparation and administration techniques. Teach to the patient signs and symptoms of diabetic neuropathy and emphasize the need for safety precautions because neuropathy

decreased sensation can hide sense injuries. Teach the patient how to manage diabetes when he has a minor illness, such as a cold, or flu. Encourage compliance with lifestyle changes, emphasize how blood glucose control affects long-term health. Advise him to wear comfortable, nonconstricting shoes and never to walk barefoot. Encourage the patient and his family to obtain additional information about Diabetes mellitus from nearby Diabetic foundations. Provide information on how important personal hygiene is. Outpatient Referrals Encourage client in complying with therapeutic regimen such as medications, diet, and proper exercise to promote wellness. Instruct to report immediately to the physician any signs of unusualities or abnormalities felt and experienced. Encourage to have a follow-up check-up with the physician after one week or as instructed by the physician to be monitored and checked.
