SCOPE OF WORK: Addendums Noted:

Section: Carpet

We will furnish the listed materials and perform the labor required for the above listed project.
This work will include Family Room carpet will be Karastan Woodside with 3/8 Crushgard pad and gold tap down
metal at two door ways.
Total Bid = $1,408.00 ALL TAXES ARE INCLUDED

This proposal excludes any take-up of existing flooring, floor preparation, moving of furniture, attic stock, weekend/night work unless
otherwise noted. Vacuuming, cleaning, waxing of resilient flooring and protection of flooring is excluded from this proposal unless
otherwise stated.

Floor preparation, including the removal of existing adhesive, curing agents, or sealers is the responsibility of the General Contractor or
Owner. If needed, preparation can be provided on a time and material basis.

RFR specifically excludes any liability with regards to existing asbestos or hazardous materials. The owner remains completely
responsible for any existing condition as they relate to asbestos or hazardous materials.

Moisture and alkalinity testing can be performed for a fee to be determined. Any remedial work required as a result of the testing can be
provided at an additional cost.

Materials will be billed upon arrival with storage affidavit and proof of insurance.

Areas should be free of all other trades people to insure proper installation.

All change orders must be in writing. We cannot proceed with any work until a signed change order is received by RFR.

A SIGNED PROPOSAL, PURCHASE ORDER, OR CONTRACT MUST BE RETURNED TO OUR OFFICE PRIOR TO ORDERING MATERIAL.

specified. Payments will be made as billed. It is understood and agreed that this is work not provided for in any other agreement.
