H2015 Individual Rehabilitation Progress Note Susanna Kaysen 12/3/2012 Goal: Member reports that she utilizes writing

as a healthy coping skill to deal with feelings of depression. Member would like to increase her writing from 2X to 3X a week. Intervention: Mental Health Worker (MHW) listened actively to ct, validating cts feelings and point of view. MHW encouraged ct to practice positive affirmations to counteract the negative messages about her writing. MHW reminded member of her previous report that writing was a healthy coping skill to deal with feelings, and praised ct for writing 2X in the past week. MHW role modeled and practiced mindfulness exercises with member, assisting member with focusing on the here and now with all five senses to assist her in better concentration in her community college class. Response: Member reported experiencing feelings of sadness as a result of her friends recent suicide. Member stated that she had written 2X in the past week, but that she had run into a mutual friend at the memorial service for her peer who had discouraged her from writing, stating artists and writers will always be poor and I dont know why you keep writing, its a waste of time. Members depression is currently a barrier to ct in focusing and concentrating in her writing class at community college. Member reports that when she does write she feels better and less depressed. Member asked MHW for assistance in practicing coping skills to deal with her depression in her writing class. Member reported a reduction in depression and increased concentration after practicing the mindfulness exercise. Plan: MHW will continue to assist member with coping skills, including writing to decrease sxs of depression. Member will continue to practice mindfulness skills to assist her in increased concentration and reduced barriers to writing 3X a week.
