INITIAL DATA BASE FOR FAMILY NURSING PRACTICE A.

Family Structure, Characteristics and Dynamics The Ibatuan Family is an extended family. A I, the husband of the client, who is 92 years old died because of pneumonia. He is the one who makes decisions for the family and also for the health before and at the same time he is the head of the family. But when he died, her wife R I, 59 years old, who is the client became the head of the family. She is the one who makes decision about their family, she is working together with her daughter R M I as a care taker

and housekeeper of a house in 136 A. Rita St. Batis, San Juan, and at the same time they are also living in the house where they are working with her niece. Her daughter R M I is 23 years old a high school graduate and works as a massager before but now a housekeeper was the one who budgets the income for expenses of the basic necessities of the family like foods, electricity, water, and for health. J I, 32 years old was the eldest child of the client which is currently living in 1213 F Castanas, Sampaloc, Manila with his family. The second child was A I, Jr., 26 years old was living in Sto. Domingo Ilocos Sur with his family. The province of the family is at Sto. Domingo Ilocos Sur. They use Ilokano as their way of communication in the house. B. Socio economic and Cultural Characteristics R I and her daughter are working as a housekeeper. They spent their money for their own expenses. R I was receiving a 1000php/month as a salary, while her daughter was receiving 3000php/month. The client verbalize Sakto naman yung sweldo namin dito buwan buwan kasi libre naman yung pagkain namin at gastusin gaya ng kuryente, tubig tsaka yung bayad naman sa renta ng bahay ay yung amo namin yung ngbabayad. According to the client they spent their money for their own expenses, Kanya kanya naman kami ng gastos kung ano mga gusto

naming bilhin sa sweldo namin, minsan yung sweldo ko pinapadala ko sa probinsya sa apo ko, di naman kasi ako masyadong magastos kasi libre naman lahat. The family usually go to the health center if they have an illness, Isang beses lang ako pumuntang health center nung nagpacheck up ako ng ubo ko pero hindi ako ngpapacheck up dun ng regular, pag may sakit lang talaga at din na kaya ng gamot na iniinom ko verbalize by the client. The client R I is an elementary graduate only while her daughter is a high school graduate and took a TESDA program as a massager. Her niece is currently in Grade 5 which is studying at San Juan. They dont belong to any ethnic background. The religion of the family is Roman Catholic. They usually attend mass every Sunday at the church near their house. The family does not attend and participate in any program that is made by the Brgy. because they are not aware about it. C. Home and Environment The family lives together in one house with other families who are renting rooms in the second floor. They dont pay any amount because they are the housekeepers. Their house is located on the ground floor and another family was staying on the second floor. The client verbalize Kasya naman kami sa isang kwarto kahit papaano. The client sleeps together in one room with her daughter and nephew. While we are doing our assessment and interview we notice that there is presence of cockroaches and mosquitoes. The client verbalize Madaming ipis dito sa bahay lalo na sa kusina namin, pati mga daga meron din. The house is made up of mixed materials. The ceiling is made up of woods with pointed nails in it. The walls are made up of woods and the floor is cemented. The stairs is made up of old wood which can be hazardous to the family. The electric plug is also not secured in the wall of the house; they just let it hung which can be hazardous to the children living in the house. They usually cook by the use of stove and sometimes they use woods. Before cooking they make sure that the food is washed well and

after cooking they make sure to clean up the used material by wiping the stove with a wet rug with soap and dry it by using a dry rug to avoid stains in it. The family is using NAWASA as their source of water for drinking and for laundry. The supply of water is adequate for them. They usually drink the water from the faucet without filtrating or boiling it. They have two toilet facilities, one in the ground floor and one in the second floor. The bowl is a manual flush type. There are also pail and water faucet inside the comfort room. They use plastic bag for their garbage and they just put it outside their house. There is a garbage truck that collects every morning for the garbage of the Brgy. They dont segregate the wastes at the form of biodegradable and non-biodegradable. They have drainage outside their house but the canal is full of plastics and other garbage. When asked about their neighbors, R I verbalize hindi ako masyadong lumalabas kasi chismosa ang maga tao diyan pero hindi naman kami magkaaway ng mga kapitbahay ko. The family is not aware about the health programs of the Brgy. The client verbalize Hindi kami umaattend ng mga program sa Brgy. kasi di naman namin alam wala naman kasi nag aanounce na meron. They use cellphones to contact their other family

members in the province. The family uses tricycle and jeep for their transportation just like when visiting their family in Sampaloc.

D. Health Status of each Family Member The patient has cough that occurs for a long time and has never consulted a nurse or a doctor but when it got worst she was rushed to have a checkup. She was diagnosed of having Community Acquired Pneumonia ( CAP ). She was prescribed by the doctor to take amoxicillin

when she consulted at the Brgy. Health Center. The daughter of the client was diagnosed to have ulcer. So they decide to minimize drinking soft drinks. Dati nagka ulcer ako kasi halos araw araw nagsosoft drinks ako, mahilig kasi ako uminom ng soft drinks pero ngayon hindi na ako umiinom, kaso kunti ako uminom ng tubig.

Wea yung P.A tsaka yung anthropometric ang ilalagay dito.si aimee ang gagawa dito. Nasa maglaya yung sunod sunod ng pgkakalagay dito. Salamats

E. Values, Habits, Practices on Health When the family was asked about their immunization status, the client verbalize Yung anak kong babae kumpleto yan ng bakuna, pero yung pamangkin ko hindi ko alam kung kumpleto yan, tapos ako naman hindi ko alam kasi dati hindi pa naman uso yung

mga gnyan. The family usually cooks vegetables for their meal. They consider it as a healthy lifestyle. The client only consulted a doctor once and never did again. She was prescribed to take amoxicillin for her cough with phlegm for 3 days which she describes as white in color. The family usually practices self medication whenever they have a fever, colds or cough. They usually take paracetamol and biogesic when they have a fever. They have their own slippers but the footwear that they used outside is the same that they use inside their house. Activity Sleep and Rest Pattern Activity Time RI
