Ashley Hamilton 4Culture Summary to Accompany Spreadsheet April 4, 2012 This summary will accompany an excel spreadsheet (the

e form) and question set intended to populate that spreadsheet. The form provided includes actual information (in black)1 and hypothetical information (in red). The hypothetical information is entirely fictional and is not intended to be representative of actual amounts; it is included only to illustrate use of the form and what eventual, actual information likely to be represented. Each column (A, B, C, etc.) will be defined and its use elaborated upon. It is my intent to provide 4Culture a means to capture and store pertinent information. This information, formally, was only provided on written and submitted program evaluation forms. In my process, these forms completely transform into electronic storage. Hopefully, the form, the questionnaire, and the King County district reporting site will allow projects to self-report and to: 1) Provide accurate district-specific presence 2) Provide funding amounts per district 3) Acquire the stories and impact of each project. 1. Elements of the Form ! Column A: Name of Project: Self-explanatory ! Column B: Contract Number: Self-explanatory; though, in my process, it was not clear if this was an office only measure or something known to the project respondents. This information can be discarded from the questionnaire if need be. ! Column C: Total Funded: this information should be known by the respondent and thus reported. Though in my initial search I did not pay strict attention to the funded amounts, I did come across receipts and such that lead me to believe this can be a valid question of the respondents. ! Column D: Origin this is the address of administration for the project. ! Columns E/F: District Location of the Origin point pre- post- district line changes the old column can be removed in the future. This new/old distinction was relevant for the initial study of how funds were shifted from the district line change. In the future one district origin column will be sufficient. Please see the Empty Sheet excel attached in the email that acknowledges this. ! Columns G/H/I :(Project Impact) Local/ Remote/Both, respectively. This is the illustration of nominal values that are signified by 0,1,2 or 3. actually had information from 2 boxes but only fully completed the Box 1 spreadsheet tab. I deleted the other tabs from the spreadsheet since they were incomplete and were not needed for the illustration of how the form could work.

o 2 = the project occurs in locations other than its Origin district location o 3 = the project occurs in both the Origin and other locations For excel, this is not mathematical calculation. The form totals only the occurrence of 1, 2 or 3 giving totals of how many projects occurred only locally, remotely, or both provided the projects indicated such. All information is provided by the program evaluations in which they specify the project location as within the Origin district or elsewhere. Columns J R: Impact Location (By District): This self-reporting element takes the local/remote distinction one step further and asks the project to define that distinction by actual location. To accurately capture this information, the questionnaire will direct the respondent to the King County district Find My District page. Here, the respondent can enter addresses to find relevant districts impacted by their services; then, report district number in the questionnaire. P.O. Box addresses are not appropriate. The estimations in red show what it could look like if respondents accurately report what districts their projects impact. Again, the nominal 0s and 1a hold as above indicating that the project occurred (1 or yes) or did not occur (0 or no) in that district. The0 and 1 allow for accurate tabulation of occurrences (provided correct reporting by respondent) across all districts. Columns S AA: Award Per District. These columns are filled by estimations of the respondent with regards to funding provided from 4Culture and was supplied to each district location. The expectation is that, by self-reporting, the dollar amounts per district can be most accurately attained. Acceptance of estimated amounts is up to 4Culture administration. The estimations in red show how funding information could be displayed if accurately provided from the questionnaire. Columns AB AH: Public Benefit (Event Type) These columns capture the type of affect or impact in the community, respectively: o Audience Participation: the audience participates or engages in the service provided. o Performance: the service to the community is a form provided by the project that does not require audience participation and demonstrates an artistic element. o Exhibit: the service is stationery; allowing audience scrutiny and assessment at will o Artifact: the audience leaves the venue with a physical element provided by engagement, participation, or attendance to the activity. o Education: the project venue explicitly intends to teach or provide a learning experience. o Child/Family Friendly: the project venue is conducive to children and family presence. o Fee Paid: If the project required a feeat any timeto attend the event. Column AI: Qualitative Data allows for the opportunity for projects to develop the story behind the project. This is a chance to identify the

importance and significance that specific project holds for the surrounding communities it serves. This is crucial since the story is the relation of the project within a context of the community it serves and from the perception of the project providers. 2. Questionnaire ! To complete the form, a questionnaire will ask project respondents to report. ! For purposes of this questionnaire, I will answer the questions as the World Music and Dance of Peace (the first project in the form labeled in blue) to demonstrate use of the form.
