 ENTRY MATERIALS
1. Each entry MUST be submitted and received as follows. Failure to send any of these
materials may nullify your application:

Two (2) completed entry forms

Two (2) executed Short Film License Agreements
Two (2) completed music cue sheets
One (1) copy of all clearance and release documents for all third party appearances and
content included in the short film
Two (2) DVD screeners of film (Note: DVDs will not be returned)
Two (2) copies of filmmaker bio and photo (have electronic version available to email)
Two (2) copies of film synopsis (max. 1 page) (have electronic version available to email)
Two (2) copies of a photo still from film (have electronic version available to email)

2. Entry title and name of entrant must be on all materials and containers.
3. There is not a limit to the number of entries: separate form required for each.
