History of Past Illness:

According to the pt. her first hospitalization was due to ulcer. She was diagnosed
of having such disease sometime in 2001. She was confined at Calamba Doctors for 3
days and undergone treatment and was then released after recovery.

History of Present Illness:

On the night of March 1 upon returning from school, the pt. started to experience
headache and from there she also developed a high grade fever. For four consecutive
days, her fever was accompanied with vomiting and diarrhea. And then her relatives
decided to confine her at Calamba Doctors but unfortunately there was no available room
at that time. The pt. was then referred to Biñan Doctors Hospital. In BDH, she undergone
series of tests and was diagnosed of having a typhoid fever.
