Procedure for Documentation at site upon receipt of Materials at Site 1. Collect all original documents from driver: a.

Invoice b. Despatch Advice Note c. Packing List d. Gate Pass (if any) e. Lorry Receipt (LR - Consignee Copy) f. Road Permit 2. Submit the above documents along with a covering letter to NTPC Site Representative for verification along with MRC 3. Preparation of MRC (Material Receipt Certificate Format Enclosed) jointly signed by NTPC and BHEL sit ein charge. 4. A Register needs to be maintained at site for all incoming materials with details including LR Number and Date, Material Description, Quantity, Date on which materials received, Road Permit Number 5. In case Road Permit is lost by the driver collect details regarding Road Permit Number, Driver name, Driver Mobile Number, Driver License Number and intimation has been given to NTPC regarding the same. Note: In case road permit is lost, BHEL has to give Rs. 1.00 Lakhs iNdemnity Bond to NTPC. Jence kindly ensure that all documents are collected from transporter and stored safely. 6. MDCC number in the MRC will be forwarded by Commercial prior to despatch of materials.
