Discourage patient to participate in strenuous activities that might precipitate stress and trauma to wound. Instructed patient to promote breastfeeding OBSERVABLE SIGNS AND SYMPTOMS Observe for dehiscence and evisceration Instructed patient to report any signs of infection Instructed patient to report any case of hemorrhage or abnormal bleeding DIET Encouraged client to increase intake of fiber to avoid constipation.
