Ophthalmic instillation Assess: External eye structure

1. Review prescribers medication order for number of drops 2. Identify the client. 3. Assess condition of external eye structure. 4. Determine whether client has any known allergies to eye medication 5. Determine either client has any symptoms of visual alteration. 6. Assess clients LOC (level of consciousness) &ability to follow directions.

7. Assess clients knowledge regarding medication therapy &desired to self administer medication 8. Assess clients ability to manipulate & hold dropper 9. Explain procedure to client. 10.Wash hands &arrange supplies at bedside 11.Ask client to lie supine or sit back on chair w/ head slightly hyper extended 12.Ask client to look at ceiling & explain steps to client. w/ dominant hand resting on clients forehead hold eye dropper approximately 1-2cm above conjunctival sac. Drop prescribed number of drops into conjunctival sac. If client blinks or closes eyes or if drops lands on outer lid margin repeat procedure. After instilling drops, ask client to close eyes gently

Instilling eye ointment 13.Holding ointment applicator above lower lid margin, apply thin stream of ointment evenly along inner edge of lower eyelid from the inner to outer canthus. Have client close eyes &rub lid lightly in circular motion 14.If client have eye patch apply clean one by placing it over affected eye. 15.Dispose soiled supplies &wash hands

16.Note clients response to medication by assessing visual changes &noting any side effects 17.Have client demonstrate self administration of next dose.
