4) Before you transport (or at the 6th District), make sure you fill out PPD Form 75-48 (Complaint or Incident Report). Fill out as much as you can and the Desk Sergeant at the station will aid you in the rest. See below on the procedure to fill out this form. a. The 48 form is required to be filled out for all arrests b. It is to be filled out in blue or black pen and be printed neatly c. The complaint is you, the arresting officer

5) If the arrestee is injured, you MUST take them to the hospital PRIOR to heading to the 9th District holding facility. If a person in police custody is transported to a hospital, a separate District Control Number (DC #) must be requested from Police Radio (via the 6th District). A separate Form 75-48 will then need to be filled out. This 48 form, along with documents from the hospital will be given to the desk officer at the 9th District. The form will be coded 3017 (Hospital Case in Police Custody) and the DC # of the original incident will be placed in the description of incident section of the Hospital Case. Fill out this form prior to leaving the 6th District as you will need them to request a new DC# for you. Do NOT delay medical treatment for an arrestee who needs it because of these forms. If you determine they need to be seen right away or if their condition worsens, take them direct to the hospital.

Make sure you put the DC# from the original incident with a brief description of the incident and injury sustained by the arrestee. Leave the rest of the form blank with the exception of the Report Prepared By section .
6) Make sure you have a copy (either one of the colored carbon forms or a photocopy) of all 75-48 forms before you head back to the park. The three colored copies usually gets distributed: a. White copy (original) retained in Operations Room b. Yellow copy (1st carbon copy) i. Report to Follow Central Detectives at 9th District ii. No Report to Follow other city agency (NPS) or district file c. Pink copy (2nd carbon copy) i. Report to Follow 6th District ii. No Report to follow 6th District when yellow copy is given to other agency 7) Please note that the sergeant on duty may have you fill out other documents or forms depending on the case.

You will park under the Whole Foods parking lot. Enter through the rear door of the police station. You will need a code to get into the first and second door. After you enter the second door you will see a lock box directly ahead. Get the key from the jailer to lock up your firearms. Then proceed down the hall to the jail to drop off your prisoner. The jailer there will assist you in the necessary paperwork. This paperwork consists of personal effects documentation and basic information forms. If necessary, you will then go talk to Central Detectives which is upstairs (through the gym) of the 9th District.
