Vidourek Program Evaluation Plan April 13, 2013 I decided to create my own health education evaluation plan. I decided I wanted to be a health specialist for young inner-city children. As an effort to help better an urban area, I decided to go out into a local school to assess and observe some childrens eating behaviors. Over the past decade or so, youth all around the U.S. have been consuming diets that are high in total energy, fat and sugar, but low in complex carbohydrates and fruits and vegetables. Unhealthy eating habits can begin early in life and tend to be taken into adulthood and as a result, chronic diseases such as heart disease, cancer, and increased weight status develop. These issues are more so a problem in inner-city youth and adults. My target population is 3rd, 4th and 5th grade students from the inner-city of Cleveland, Ohio at East Cleveland Middle School. My needs assessment will be given to the children at the local elementary school. I would also give it to the parents, teachers and the principals of the students. The needs assessment would consist of no more than 10, easy-to-read questions for the students and a different assessment for the adults so I could get a better understanding of what the students eat, if they are active and how they spend their free time, at home and at school. Once I receive all the information needed, I planned to have an after school program for the students. All the results from my survey helped me to make my decision for my school-based educational program. I asked a number of background questions that would assist me in building an appropriate program for the young students. The program will meet every other day (Mon., Wed., Fri.), after school for one hour, for the selected grades for six weeks. With the consent and

cooperation of the parents and teachers, I hope to better a childs health by teaching them to eat right and stay active. I used tools such as power point presentations, handouts, coloring sheets, posters, fruit and vegetable models and outside active games. Program Objective: One year after the program, 80% of the participants will have tried a new type of food they have never had before. Behavioral Objectives: One year after the program, 80% of the participants will have tried a new type of food they have never had before. 7 months after the completion of the program, 50% of the participants who completed 85% of the program will reduce their intake of junk food. Learning/Knowledge Objectives: After watching the video, Eating your Fruits and Veggies during the third meeting, 30% of the program participants will be able to list 3 different vegetables. After the lecture of Eating Properly, during the sixth meeting, 40% of the participants will be able to identify the difference between a zucchini and cucumber. After the power point presentation on how fruit and vegetable servings, during the fourth meeting, all program participants will be able to list the correct amount of daily fruit servings. By the end of the program, 75% of the participants will be able to correctly read a food label.

By the 4th week of the program, 80% of the participants will be able to list all the food groups.

Educational Objectives: Participants will be given a short quiz to assess their knowledge on healthy eating. Participants will be shown pictures and fact sheets about the risks of poor dieting. Participants of the target population will be given educational activities and environmental change activities. Participants will listen to the guest speakers personal testimonial with living with diabetes as a result of not eating right. The participants will play indoor and outdoor activities that will keep them active throughout the year. The evaluation plan that I will use for the process evaluation is a checklist and a post program meeting with key informants. The checklist is used as an evaluation for me, the administrator, making sure to check off my tasks as I complete them. I will hold the post program meeting with key informants such as the parents, teachers and the principle of the students on Saturday, June 22, 2013, two weeks after the program ends one week after the postprogram survey completion, to determine which mechanisms of the program worked successfully and what needs to be improved. I will only be measuring myself through the checklist and by conducting a small focus group with the key informants. The overall evaluation has helped me to piece together the process of program planning, implementation, and the evaluation. After the completion of the program and after reviewing the

entire program, I can check to see what areas of the program were successful and what areas need to be worked on for future programs. For my behavioral objective I choose to explain that 7 months after the program is over, 50% of the students, who completed 85% of the program, will have decreased their intake of eating junk food. The who, is talking about the students, who completed to program. By how much is 50% and by when is seven months after the program is over. When the objectives are measured, during the process stage, educational objectives were measured. During the impact stage, behavioral and learning objectives are measured and in the outcome stage my goal is measured.
