Running head: SERVICE LEARNING PROJECT PLAN Your name: Daniel DeLauter

Date: 11/24/13

Community or population of interest: The Frederick Rescue Mission service homeless and low-income families within Frederick County, MD. The Frederick Rescue Mission is a Christ centered facility that serves the population of Frederick city and its surrounding areas. On a daily basis, they serve 365 meals to homeless and others in need. The also provide a recovery program to residents that sign on to a 12 month program. The mission also provides clothing to homeless and impoverished people. The Frederick Rescue Mission seeks to rescue lives from poverty, homelessness and chemical dependency. We exist to meet the needs of hurting and broken people while inviting them into the abundant life Christ offers to all. Every man that enters with a heart surrendered to God and a cooperative attitude should never have to see a prison, jail, homeless shelter, rehab center or rescue mission again. He should never have to sleep under a bridge, in an abandoned house or out in the woods again. A changed life will call on Jesus instead of alcohol, cocaine, heroin, and any other kind of drug. The goal here is to see lives changed for eternity, now and forever (therescuemission.org). According to the National Alliance, Maryland had 9,623 homeless persons in 2007 (DHCD 2007).

Review of literature and health outcomes data: Three of the top health concerns for the homeless population are: bronchitis or pneumonia; substance abuse; and wound and skin infections (NIH). National Institutes of Health. (2013). Homeless health concerns. Medline Plus. Retrieved from http://www.nlm.nih.gov/medlineplus/homelesshealthconcerns.html#cat10.

SERVICE LEARNING PROJECT PLAN In 2007, study was performed to improve the nutritional status of homeless children. The study implemented four classes for parents. The studied showed that the classes to improve nutritional selections helped parents to have the knowledge to select more nutritious food. However those selections were not made due to lack of supply at the homeless shelter (Janken). Janken, K.; Leake, J.; Wdowik, M.; & Yousey, Y. (2007). Education in a homeless shelter to improve the nutrition of young children. Public Health Nursing (3) 249-255. Charlotte, NC: Blackwell Publishing In 2007, a randomized study showed that 6.2% of the U.S. population is homeless. Toro, P. et al. (2007). Homelessness in europe and the United States: A comparison of prevalence and public opinion. Journal of Social Issues. (63). 3. 505-524. Blackwell Publishing.

Your selected health concern focus: Federal nutrition assistance programs. Education of nutritious food selection

Three possible nursing interventions: Post flyers on information regarding federal nutrition assistance programs. -Eligibility resource at pre-screening tool on http://www.fns.usda.gov/snap/eligibility. At the core of the FNS mission is our responsibility to provide access to nutrition assistance program benefits to every eligible person who needs and seek assistance. The major nutrition assistance programs, including the Supplemental Nutrition Assistance Program (SNAP), the Child Nutrition Programs, and the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), are the first line in the Nations defense against hunger and poor nutrition. Too often people who need help do not participate in SNAP or WIC because they do not know they are eligible, do not realize the size and value of benefits available to them, or find applying too difficult or burdensome. Similarly, not all eligible children participate in the school meals programs. Some bring healthful food from home, but others may forego a nutritious lunch entirely due to lack of understanding about program eligibility requirements (USDA).

SERVICE LEARNING PROJECT PLAN Conduct an information session regarding different nutritional assistance programs. Tour of the http://www.fns.usda.gov/ At the core of the FNS mission is our responsibility to provide access to nutrition assistance program benefits to every eligible person who needs and seek assistance. Post information regarding nutritious food selections. -Homeless people are more at risk for wound infection. Improved nutrition aides in the healing process therefore lowering wound infections. Your selected nursing intervention: Post flyer regarding different nutritional assistance programs available to U.S. citizens

Brief plan for conducting nursing intervention: Briefly describe the benefits and ways to access applications to the Women Infant and Children program (WIC), the Supplemental Nutrition Assistance Program (SNAP) and the Summer Food Service Program on a flyer and post them in and around the Mission. This will spike the interest of people in need within the Rescue Missions. The Mission serves breakfast and lunch. It does not serve dinner. These programs will help supplement people nutritional needs in the evening therefore improving health. If a follow up intervention such as an informational session due to high interest, that may be conducted.
