Agreement between the employer; True Fitness and the company employees
Terms and Agreements are stated as follows:
1. Personal Trainers Sale and Purchase Document
- All employees are required to fill all information needed in Personal Trainers Sale and Purchase
Document and Personal Training Rule and Policy Reviewing Form completely including a customers
signature, an employees, and a department managers. If any document is not completely filled or invalid, it
will be considered CNI automatically.
2. A customers name in Personal Trainers Sale and Purchase Document as well as a receipt and a credit
card used for Personal Training fees must completely match and be accurate.
- Providing the information in Personal Trainers Sale and Purchase Document and a credit card slip used
for Personal Training fees fails to match or invalid and/or a credit card slip doesnt show the customers
name, the employee must copy the customers ID card and his or her credit card (both sides) and ask the
customer to sign his or her name stating that the document copied is valid and he or she allows the company
to deduct the payment with their credit card for Personal Training fees. If the requirement fails in any part of
the document, we will consider that this Personal Trainers Sale and Purchase Document or related
document is invalid and incomplete as CNI.
3. Once Personal Trainers Sale and Purchase Document is completed done according to the requirements,
the employee is required to direct the customer to confirm his or her purchase and validate all document in
an Operation Department. All customers are strictly required to sign his or her name in all document
including Personal Training Rule and Policy Reviewing Form with the signature evidence or witness of an
Operation Department. If there is no signature evidence or witness of an Operation Department, the
company will consider the document is invalid and incomplete as CNI.
4. Due to the previous CNI document submitted earlier, the company will allow the employees to fix and
complete all CNI document as well as to re-submit it within the 5th of the following month. If any employees
fail the company requirements namely they dont fix the document or re-submit on the due date, the
company will not pay any commissions in a consideration of an invalid document terms.
Providing any employees dont follow the requirements or attempt to break the rules mentioned in this
agreement, the company will consider that employee is performing against the company policy and regimen
which he or she may encounter the punishment according to the company rules and regulations.
Both parties officially sign to accept and follow the terms and agreements mentioned in this agreement.
