Queens College Careers in Healthy Alumni Mentoring Program Mission

Queens Colleges Careers in Health Alumni Mentoring Program (CHAMP) is supported by the Queens College mission Discimus ut Serviamus: We learn so that we May Serve. With our dedicated alumni representing MDs, PhDs, DDSs, and DMDs, we aim to develop and support our undergraduates in their pursuit of professional career paths in health related/medical fields. This mentoring relationship will allow participating students to clarify their career interests and goals and will be invaluable in their personal development. The structured mentor-mentee relationship will last for the duration of the students undergraduate career. By extending the length of the structured relationship between mentor and mentee, we hope to create a more efficient system that will support students in ultimately achieving their professional career goals. In the subsequent pages you will find an agreement for CHAMP mentees and CHAMP mentors. This contract is meant to serve as a framework for meetings. This contract should be discussed, completed and signed at the first meeting with your mentor.

FAILURE TO SUBMIT THIS DOCUMENT WILL RESULT IN TERMINATION FROM THE PROGRAM. This agreement serves as a guideline for Mentors and Mentees. The students objectives and goals will be accomplished over the course of program. The Goals and objectives should be updated as the program progresses.
