COURSE IN THE WARD

From September 11, 2013 to September 14, 2013 Day 1: 9-11-13 At 10:38, Patient was admitted to ICU with an admitting diagnosis of HCVD, CAD. VS was taken with the following results 140/80mmHg, 88bpm, 20bpm, 36.1C CBC, CBG test were ordered by the physician. Stat med was given: Clopidogrel 300mg PO Day 2: 9-12-13 The patient was received awake. BP was taken, 130/90mmHg. On CBC ,CBG monitoring BID. With low salt low fat, diabetic diet. Day 3: 9-13-13 The patient was received awake. On low salt, low fat, diabetic diet. BP was taken 120/90 mmHg. Transferred to regular room.

Day 4: 9-14-13 The relatives of the patient requested for HAMA(Home Against Medical Advice). The patient was seen by the physician and ordered HAMA. Staff nurse on duty explained clearly the advantages and disadvantages of HAMA. The consent was signed and discharge planning was done.The patient was discharged per wheelchair, on HAMA, accompanied by relatives.
