Administer initial vocational assessment. 1 Discuss the following at IEP meetings: 1 - Academic - Social - Language/Communication - Occupational - Self-help skills - Self-advocacy skills Develop and implement strategies to increase responsibilities and independence at home. 1 Complete periodic vocational evaluations. 1 Introduce and discuss transition services. 1 Notify parents that transition services will be incorporated into the IEP beginning at age 15. 1 Make sure that copies of work-related documents are available: 1 - Birth certificate - Social security card - Obtain working papers (if appropriate) Obtain parental consent so that they appropriate adult agency representative(s) can be involved. 1 Develop transition component of IEP and annually thereafter. 1 Discuss high school diploma track options (decision must be made before student enrolls in first semester of high school classes). Review appropriateness of diploma track after both first and second semester of freshman year. Review records for documentation of disability. Evaluations must be current for ACT/SAT accommodations, and colleges will expect documentation no older than 3 years. Discuss adult transition. 1 Consider summer employment/volunteer experience. 1 Explore community leisure activities. 1 Consider the need for residential opportunities, including completing applications, as appropriate.

Develop transportation/mobility strategies: 1 - Independent travel skills training - Alternative transportation Investigate SSDI/SSI/Medicaid programs. 1 Consider guardianship or emancipation. 1 Develop and update employment plans. 1 Involve VR, as appropriate, within 2 years of school exit. 1 Research possible adult living situations. 1 Investigate post-school opportunities (further educational vocational training, college, military, etc.). 1 Students interested in the military should meet with guidance counselor (or other appropriate staff member) to discuss the Armed Services Vocational Aptitude Battery (ASVAB). Seek legal guardianship. 1 Apply for post-school college and other training programs. 1 Male students register for the draft. 1 Register to vote. 1 Review health insurance coverage (inform insurance company of students disability and investigate rider of continued eligibility). 1 Create records of documentation regarding disability for future post-secondary and/or employment needs. Create an action plan for accessing necessary services in next setting.
