is to showcase the students involvement with various community partnerships, which includes the utilization of the Health Planning Model to improve aggregate health through the application of the nursing process.\ Student Approach to Assignment The aggregate that I worked with in preparation for this paper was a group of ten African American women that do not have sufficient income in order to live on their own. They were all chosen by referrals and an extensive screening process and were required to work or go to school or the entirety of their 2-year stay. Through working with this aggregate and my classmates, I acquired a unique nursing experience by providing preventative care within the community instead of a more traditional acute care experience. My group and I approached this assignment by coordinating activities throughout the semester in order to get to know the women, gain their trust and survey learning and heath needs that they had. Reason for Inclusion of the Assignment in the Portfolio This assignment is included in the portfolio because it highlights a different aspect of nursing care provided, including the work I did with the community, which exemplifies leadership and teamwork abilities and community partnership initiation. The following program objectives are highlighted: Critical Thinking Engages in creative problem solving. Example: As part of our first semester in community health in which we planned interventions for our aggregate, we identified powerlessness as a significant trend. In order to help identify sources or contributing factors of powerlessness or other personal life stressors, we collectively decided to implement Photovoice as a creative means of expression. Photovoice is the use of taking pictures that represent significant parts of a persons life and making a collage or adding captions. We thought this would help the women of the house acknowledge, and then begin to accept and be rid of the negative memories or characteristics of their pasts. Teaching Evaluates the efficacy of health promotion and education modalities for use in a variety of settings with diverse populations. Example: During the time spent with this aggregate I participated in continual evaluation of each womans individual mood and verbal and nonverbal indications of hopelessness and their willingness to interact with us in order to
evaluate the efficacy of implementing Photovoice. These women, although living together in one house, came from a variety of backgrounds and each had a different level of interest in the project. Example: As a group we surveyed the food kept in the house and then informally educated the women on what was unhealthy and what a healthier choice would be that was already in the house. I evaluated based on the attitudes of the women towards this information whether or not they would respond positively to an exercise and healthy eating regime. Most seemed to be eager to participate as the reason for poor food choices was lack of knowledge. Leadership Assumes a leadership role within ones scope of practice as a designer, manager, and coordinator of healthcare to meet the special needs of vulnerable populations in a variety of practice settings. Example: At some points during the course of this project I took a leadership role by coordinating healthcare to meet the needs of our aggregate. For example, at the start of the project I coordinated the completion of the windshield survey for our aggregate and complied the information for reference for this paper. Example: I also served in a leadership role during an intervention to get to know the women of Barrett House in which we did yard work around the house and planted some flowers. During this activity, I coordinated tasks by delegating who would mow the lawn, who would rake leaves, etc Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals. Example: By establishing a community partenership with these women, we were able to identify needs of the women and establish health goals and implement strategies such as using Photovoice as a means to meet the goal of decreased powerlessness and improved sense of self-confidence and selfworth. Organizes, manages, and evaluates the development of strategies to promote healthy communities. Example: I helped to organize and manage the development of strategies such as Photovoice to increase sense of empowerment and healthy eating habits to increase health and energy. We planned to evaluate these strategies by periodical emotional state checks and nutritional assessments such as monitoring eating habits throughout our time with them. Culture Maintains an awareness of global environmental factors that may influence the delivery of healthcare services.
Example: As part of this paper I assessed the environmental factors affecting the women of Barrett House and the availability of healthcare and other services to them. Globally, the neighborhood in which Barrett House is situated is rather low income and there are no grocery stores or healthcare services within walking distance of the house. Very few of the women have cars or money for bus fair. With constant awareness of this in mind, we planned to implement healthier eating habits with the foods that are donated and delivered to the house, for example.
