Final IEP Project- Denelle Cramer

Lola is an 8 year old girl who participates in the Special Education classroom for the majority of her educational experience. Her grade by age is second grade, but her academic level is that of a kindergartener. Her IEP goals are a product of her diagnosis of mental retardation and learning delayed. Lola enjoys school and she enjoys participating in music, Physical Education, and recess with her general education peers. However, Lola struggles with personal hygiene needs and which affects her ability to be truly accepted and included in her peers activities. Lola has been in the same school for four years, as she initially qualified for developmental preschool at the age of four. She has made a lot of progress in social skills and having appropriate behavior in the classroom. Her ability to receive instruction has improved through the meeting of her prior IEP goals which included the following: Sitting and listening for 15 minutes at a time. Going to the bathroom independently. Appropriately participating in group activities. Basic art activities such as gluing and taping items on paper, adding textured elements such as cotton balls and feathers. Building structures out of blocks and manipulatives. Hanging up her coat independently in the morning. Feeding herself at lunchtime and snack. Waiting for a prompt to begin an activity. Counting to 5.

Eligibility Statement Lola is a second grader by age, but academically she is still in Kindergarten. Lola has been diagnosed as having mental retardation and learning delayed. These disabilities affect her academically and socially, so consequently, Lola needs an IEP to outline very specific goals and benchmarks in order to teach her effectively.

Annual Goal Target date November 14th, 2014 3 month benchmark target date February, 14, 2014 6 month benchmarks target date May 14, 2014 9 month benchmarks target date August 14,2014

IEP GOAL ONE Lola will be able to identify ways to promote personal hygiene by way of initiating her own healthy behaviors such as brushing her teeth after eating, combing her hair and washing her hands after using the bathroom. The special Education teacher will assess her daily by prompting her to do these things as necessary and to create a poster chart where Lola can earn a star every time she does these things.

State Standard K-2.H.1.1.1. Identify that healthy behaviors affect personal health.

Present level of performance Lola does not have the guidance at home to come to school with her hair combed and her teeth brushed. 3 month benchmark: Lola will be able to follow directions to get her personal hygiene basket in the classroom bathroom, and comb her hair and brush her teeth. She will finish and put her things away. 6 month benchmark: Lola will be able to conduct her personal hygiene without prompting when she arrives at school. 9 month benchmark: Lola will be able (by way of education her mother on the importance of conducting personal hygiene activities at home) to come to school with these things already done.

Lolas teacher will create an achievement poster, where Lola can earn stickers to put on her poster as she completes the personal hygiene activities for the day. Her teacher will prompt Lola less and less as she advances closer to being able to prompt herself to carry out these activities on her own.

IEP GOAL TWO Lola will be able to identify and classify objects by shape and color, and be able to group them and describe how many sides the objects have and what color they are. (ie: squares, triangles, circles) Also Lola will be able to count the shapes up to 20. State Standard K.MD3. Classify objects into given categories; count the numbers of objects in each category and sort the categories by color and shape. Present Level of performance Lola sees the shapes as being objects of play. She currently does not recognize the different shapes. She recognizes the color black and red with 95% accuracy, but tends to call other colors those as well. She currently can count the shapes up to 5. 3 month benchmark: Lola will be able to recognize colors blue, white, yellow, red, black, and green with 25-50% accuracy. She will be able to recognize and sort circles, squares, and triangles, with 25-50% accuracy. 6 month benchmark: Lola will be able to recognize colors blue, white, yellow, red, black, and green with 50-75% accuracy. She will be able to recognize and sort circles, squares, and triangles with 50-75% accuracy. 9 month benchmark: Lola will be able to recognize colors, blue, white, yellow, red, black, and green, with 75-% accuracy. She will be able to recognize and sort circles, squares, and triangles with 75-95% accuracy. In the event that Lola meets her IEP goal early, rectangles will be implemented into the group of shapes, and she will work on recognizing the difference between squares and rectangles. Lola will practice identifying shapes and colors by integrating these concepts into

her daily lessons. For instance, if p is the letter of the week, she will listen to a story about pizza, or if the letter of the day were d she would listen to a story about doughnuts (circles), her art project might be to make a red pizza out of clay, or to draw a circle and decorate it with cheerios, and her manipulative activity will be to find the circles in the bin of blocks. IEP GOAL THREE Lola will be able to draw pictures and narrate the story of her picture, in a chronological order of events. Lola will be able to discuss the feelings associated with the characters and events of her story with her classmates and teachers as her audience. Her special education teacher will read simple stories and describe events and feelings associated with the events, and assess Lola by initiating discussions about the story. State Standard WS-3: Use a combination of drawing, dictating, and writing to narrate a single event or several loosely linked events, tell about the events in the order in which they occurred, and provide a reaction to what happened. Present level of Performance Lola can draw unrecognizable pictures, and describe them individually, without identifying order or meaning. She enjoys drawing and describing her pictures, but does so with little ability to tell a story. Lola recognizes the general feeling of the story, but has little ability to describe the story. 3 month benchmark: Be able to listen to a story read aloud by her teacher, and participate in a discussion with her classmates about the overall feeling of the story and be able to identify who the story is about with 25% accuracy as determined by her teacher. 6 month benchmark: Lola will be able to recognize events and feelings in the story with 50% accuracy. Lola will be able to describe the events of the story to her teacher and her classmates, in chronological order with 25% accuracy. 9 month benchmark: She will be able to recognize the feelings associated with the story and be able to verbally portray her interpretation of the story, with 75% accuracy. She will be able to put events in chronological order with 50% accuracy. In the event of successful completion of her ability to identify feelings and order of a story with the percent of accuracy identified in the 9 month benchmark, she will continue to work toward 90-95% accuracy in those areas. She will also start to begin to identify the causes and effects of the events in a story.

Her special education teacher will read stories aloud and initiate discussions about events and feelings. She will do this as a group activity, in order to allow Lola to hear the responses of her classmates, and to prompt Lola to participate. Her teacher will implement activities such as having Lola draw pictures and describe the feelings and events associated with her story to her classmates. Lolas teacher will assess her in this area by comparing her ability to do this from week to week, and documenting her growth in this academic area.

Remaining IEP Elements The most appropriate environment for Lola will be in the Special Education classroom for most of her learning. She will, however, participate in music and physical education with her general education peers, as she enjoys the social aspects of those activities. Lola will require the services of special education transportation, as she has no older siblings to help her on a general education bus. Lola will require the services of a Speech Pathologist and an Occupational Therapist. No other specialized therapy will be required.
Lola will not take part in state or district wide assessments for her grade due to her

disabilities but will be tested accordingly, through her annual goals identified in her IEP. This IEP will begin immediately and end in one calendar year. If success comes before the end of the year, the IEP will be modified at that time.
