The Philippine Records Management Association (PRMA) now offers you the ability to complete and save

the Registration Form directly on your computer.


When used with versions of Acrobat Reader, you may enter data in the form and save it, print it, or
email it. The latest version of Acrobat Reader is available as a free download from www.adobe.com.

1. The PRMA Registration form is available through Scribd which is a File Hosting Website. You must be
logged in to your Scribd account before you can download the PRMA Registration form from the site; if
you're not logged in, you'll be prompted to do so before your download begins.

If you have not signed up, don’t worry – sign up is absolutely free. All you need is a valid email
address (e.g. Google account (Gmail), Yahoo account, FaceBook ID, etc.)

Please login or sign up to get to the screen below.

2. Click this PRMA Registration Form link. When you’re viewing the form, you can find the Download link
on upper left corner of the screen.
3. When you click Download, it will expand to show you the format(s) in which you can download the
document. Right-click on the Adobe Acrobat (.pdf) link and you will get a dialog box prompting you
to open or save the file.

4. Select Open or Save from the File Download window. When you choose Open, you are ready to fill up
the form. When you choose Save, the document will be saved directly to your computer or to a disk.
You may complete part of the form, save it, and reopen it at a later date to modify or add to it. Make a
note of the location on your drive and the name of the file being saved so you can locate it later.
Use Acrobat 5.0 (or later) to open the file.

In some cases, your browser may require you to right click on the link, then select "save link as" or
"save target as" from the dropdown options.

5. After opening the form, position the pointer/cursor inside a form field and click. The I-beam pointer
allows you to type text. The arrow pointer allows you to select a field, a check box, a radio button, or
an item from a list.
6. To accept the field change and go to the next field, press Tab or use your mouse to click on the next
form field.
7. To accept the field change and go to the previous field, press Shift + Tab or use your mouse to click
on the previous form field.
8. Click the Submit by Email button after completing the form so that it can be sent to PRMA.
