Problem
Cause Recomendation Theres no problem in collecting data mechanism We divided our team into 5 groups each responsible to collect data for M1 until M5. We Use data that already in room, questionnaire instruments, observation and interview the nurses to support our data
Content Problem
Cause Recomendation Documentation form of instruments was uncompleted. Showed by the inventories book that hasnt been filled by the new data of current instruments.
Chief of decomentation is newly formed, so there was not a documentation before.
