I, the undersigned, Parent/Legal Guardian of ______________
___________________, hereby agree to indemnify and hold harmless the church, its
employees and volunteers, its governing board, the individual members thereof, and all
other officers, agents and employees from any liability, lawsuit, cost, expense, or claim
of any type whatsoever (including legal fees) for any harm, injury or death arising out of
the above mentioned activity, as a condition of my child/youth participating in the same.

Permission for Activities with Restoration Baptist Church / Authorization to Treat Minor
As legal guardian, I give my consent for the student listed above to participate in the
Sunday school, Kids Church, and Youth group, and other ministry event/activities
provided by Restoration Baptist Church; including transportation in ministry or personal
vehicles. I have provided all necessary medical and personal information for my child. In
the event that I cannot be reached in an emergency, I hereby permit the concerned
authorities (ministers, church leaders, youth workers) to call 911 and/or to contact a
medical facility or physician selected by me to provide proper treatment to

Optional Photo Release
Minor children (under age 18) need to have a parent/legal guardians permission for
Restoration Baptist Church (RBC) to use any picture in which they appear on the RBC website or
in other materials depicting minor children in Sunday School, Kids Church, and Youth activities.
If you mark yes, the undersigned party gives permission for Restoration Baptist Church to use
such pictures. If you do not want your childrens pictures on the website or other church
materials, please mark no on the space provided. In both instances, please sign your name
below.
Please note that if you select no, we may have to ask your child to not be included in any
photography being done. Please explain this in advance to your child. Restoration Baptist
Church has permission to use any or all pictures in which my child appears for the RBC website
or printed materials.
