Lesson Times: I try to work with each of my students in order to give them the most convenient lesson
tme. Please be punctual to lesson; if you arrive late, I stll must end on tme.
Locaton: Unless otherwise informed, all private and group lessons will take place in my home.
Illnesses: Students who are sick should not come to piano lessons. The extra group classes serve as
make-up opportunites. Students can also send videos of their songs for me to assess when absent.
Late payments: Payments made afer the 10th of the month will be assessed a $10 late fee.
Recitals: I expect all students to atend recitals.
Practce and other requirements: In order to make progress and contnue to advance, my students are
required to practce. Parents should read their childrens assignments and comment on/inital them
each week. Parents should also plan to atend their childs lesson at least once every 3 months.
Dismissals and terminaton: If for any reason you must stop lessons, 1 months notce or 1 months pay
is required. Regretably, I will have to dismiss a student if any of these circumstances occur: persistent
absences, consistent lack of practce, uncooperatve or disruptve behavior, contnued failure to pay
tuiton on tme.
Photo/video releases: Please see the photo/video release form.
Missed lessons: Because specifc tmes are set aside for specifc students, a student who does not
atend a lesson will simply miss that lesson and I will use the tme to plan for the students progress
and prepare materials for them, etc. You are paying for the whole session, not each individual lesson.
There will be three group classes spread throughout the year that can be viewed as a form of make-
up lessons, since I do not charge for group classes.
60 minute lesson/lab split
Students will receive a 30 minute private lesson plus 30 minutes of lab tme each week. The private
lesson and lab will be scheduled back-to-back and the lab tme is during another students lesson. Dur-
ing lab tme students will be learning about music theory, music history, doing ear training, composing,
and much more! The overlap with another student also ofers opportunites to
work on duets throughout the year. Tuiton: $90/month
30 minute private lesson
Students receive a half-hour private lesson each week. Tuiton: $ 80/month
45 minute private lesson
Students receive a 45 minute private lesson each week. Tuiton: $120/month
Tuiton is based on 32 teaching weeks and is divided into 10 equal monthly payments of the above
amounts. The tuiton includes all materials. Therefore the tuiton is the same amount each month
regardless of the number of lessons in that month.

It is a joy for me to teach piano to my studentsI love what I do! And it is important to me to pass that joy of music along to each of my
students. However, it is a team efort that includes myself as the teacher and you the parent as supporter...as well as the student.
Learning the piano is a challenging and rewarding adventure. It takes tme, patence, and diligencebut it is well worth the efort. Since I
only see each student once a week, it will important for parents to help establish a positve and productve practce environment at home.
With regular practce students progress steadily and lessons are more enjoyable and fun! (I have never met a child yet who doesnt like
piano, but I have met plenty who aren't self-disciplined yet:). So in order to make my tme as a teacher more efectve and keep things
running smoothly, I have established the following policies:

AUDIO/PHOTO/VIDEO MEDIA RELEASE FORM
I do hereby give permission for my child/ren to be photographed or recorded for promotonal
materials including fyers, web site/blog postngs, video postngs, or in-studio collages in con-
necton with Keys To Success Studio. I do so without expectaton of compensaton & with the
understanding that these photos will be used exclusively by Keys To Success. I also
acknowledge that Keys To Success may choose not to use my photo at this tme, but may do
so at its own discreton at a later date. I agree to indemnify and hold harmless from any
claims Rachel Luecke or Keys To Success. I understand that Keys To Success will not use my
childs full name in postng any photo or video.
I hereby agree to release, defend, and hold harmless Keys To Success and its agents or em-
ployees from any claim, damages, or liability arising from or related to the use of the photo-
graphs/video, including but not limited to any misuse, distorton, blurring, alteraton, optcal
illusion, or use in composite form, either intentonally or otherwise, that may occur or be pro-
duced in taking, processing, reducton, or producton of the fnished product, its publicaton,
or distributon.
As the parent of the child/ren listed below I have read this release before signing and fully un-
derstanding the contents, meaning, and impact of this release. I understand that I am free to
address any specifc questons regarding this release by submitng those questons in writng
prior to signing, and I agree that my failure to do so will be interpreted as a free and knowl-
edgeable acceptance of the terms of this release.

About the photo/video release form.
I like to video my students playing so they can listen to themselves or send the video to family and friends. I
also record student recitals and other events and achievements, and I take pictures to put up in the studio or
to highlight on the website. I am very careful with what I post online and usually check again with parents
before postng something, but I stll have to have a photo/video release on fle. If you have any questons or
