How to Use

Google Forms
provides an easy way to
create online surveys,
quiz, polls, and other
similar forms.
Create your survey and
invite respondents by
email.
People may answer your
questions using any web
browser.
The responses will then
be collected in an online
spreadsheet.
Best thing of all, its free!
In this tutorial, you will
learn the basics
of using a Google Form to
make a survey.

Google Forms
To access Google Forms
you should have an
account on any Google
application.
From any Google
application home page
click on the Apps icon on the
upper right part.
Then click Google Drive.
This is the Google Drive window.
Click Create
and click Form.
Google Form window appears.
Click on this part
and type in the Title of your
form.
Click on this optional field
and type in a description of the
form.
How to Add
Questions, Headers,
and Page Breaks
Questions
Once you have created
your form
you may now add
questions to your form.
There are 9 types of
questions offered in
Google Forms.
1. Text - respondents
provide short answers
2. Paragraph text
respondents provide
longer answers
3. Multiple choice
respondents select one
option from among
several
4. Checkboxes
respondents select as
many options as theyd
like
5. Choose from a list
select one from a
dropdown menu
6. Scale respondents
rank something along a
scale of numbers
7. Grid respondents
select a point from a two-
dimensional grid
8. Date respondents use
a calendar picker to enter
a date
9. Time respondents
select a time (or duration)
The field for your first question
is already given.
Click the down arrow of
Question Type
and choose the type of your
question.
Then type in here your question.
You could put some help text
here.
Check this box to make sure that
users
answer the question before
submitting the form.
Click Done.
Youve made your first question!
Click on the down arrow of Add
item
and choose what type your next
question is.
Again, type in your question
here.
and type the options in these
fields.
Click the x to remove an
option.
Click Done if satisfied.
The 2
nd
question is added to your
form.
To edit a question, mouse over
the question
and click the Edit (pencil) icon.
Edit the desired field.
Click Done.
The question is now edited.
You may also duplicate a
question.
Mouse over a question
then click the Duplicate icon.
Same question appears
that you may edit accordingly.
Click Done when satisfied.
The question is then added to
your form.
To remove a question, mouse
over a question
then click the Delete (trash bin)
icon.
The item is removed from your
form.
Section Header
You may also add headers
and page breaks
to make your form more
structured.
To add a header
click the down arrow of Add item
Then click Section header.
Type in here your Header text.
Type in here your description.
Click Done.
Section Header is added to your
form.
Add questions in your
new section
like what weve done in
previous slides.
Page Break
To add Page Break, click
the down arrow
of Add item.
Then click Page Break.
You can type in here the title of
your page
and description as well.
Click Done when satisfied.
Your new page is added.
In the live form, your first page
will have
a continue button at the
bottom.
The next page appears
upon clicking continue.
How to Add an Image
and Video
Image
You may also add an
image to your form.
To add an image, click the
down arrow
of Add item
then click Image.
This window appears.
Click Choose an image to upload.
Click your image file.
Click Open.
You may enter here your Image
Title
and the text that will appear
when hovered on.
Click Done.
The image is added to your form.
You may drag this image
anywhere in your form.
Video
You may add a Youtube
Video on your form.
To add a video, click on
the down arrow
of Add item
then click Video.
This window appears.
Type your search in the box to
find videos.
Click Search.
Click your desired video.
Click Select.
Give your video a Title here.
You may also give a caption here.
Click Done when satisfied.
The video is added to your form.
You may drag your video
anywhere in your form.
In the live form, your video looks
like this.
How to Add a Theme
To add a theme to an
existing form
Click Change Theme in the tool
bar.
Themes appear on the right side
of the window.
Click your desired theme.
The theme is applied to your
form.
If you want to customize even
more, click Customize
below the theme template.
From here you could customize
every part of your form.
Just click on your desired part
then edit.
Your form is now customized.
Controlling
Page-to-Page
Navigation
If you have divided your
form into different pages
you can specify which
users see which pages
depending on their
answers to questions in
your form.
sort of like a
Choose Your Own
Adventure
story.
To send users to a specific
page
based on their answer to a
certain question

select either
Multiple Choice
or
Choose from a list
type of question.
Only these 2 question
types can be used
to direct users to specific
pages.
No other question type
can.
For example, click either a
Multiple Choice
or Choose from a list
question in your form.
Check the box of Go to page
based on answer.
Continue to next page fields
appear for each option.
Click Continue to next page field
and choose your desired page in
your form.
Do this for each option.
Click Done when satisfied.
The respondent will then
be routed to a specific
page
depending on their
answer to that certain
question.
Confirmation Page
Settings


Confirmation Page is the
one you see
at the bottom of your form.
This is also the page users
will see
after submitting responses
to your form.

Click here to change the
confirmation message.
Check this box to allow users to
submit
as many form responses as they'd
like.
Check this box to give
respondents
access to the form's summary of
responses.
Check this box to allow
respondents
to change their answers to your
form.
How to Send Form to
Respondents


Click Send Form on the
Confirmation Page section
or on the upper right corner of
the Forms window.
This window appears.
You may copy your forms link
here
and share it with others.
You could also share the link in
Google Plus
Facebook
or Twitter.
If you click Facebook, for
example
Google Forms will direct
you to Facebook website
to make a post containing
your link.
You could also send form via
email.
Enter the names or email
addresses here
Enter in your message here.
Then click Send.
If you like to embed your
form in your website
Click Embed.
Copy your forms HTML
and paste it in your website to
embed the form.
Click Done to leave the page.
Form Response
Destination

After youve done your
form
and have sent it out to
your respondents
Google Forms will begin
collecting their responses.

You may see the responses
in a spreadsheet
and in a CSV file that you
can download.
With the spreadsheet,
you will be able to see
individual responses
as they come in.

The downloadable CSV
file will let you
see a summary of all the
responses youve received.
Responses in the
Spreadsheet
Click Responses in the tool bar.
Click Choose response
destination.
Click New spreadsheet
then click Create.
To view your spreadsheet
click View responses in the tool
bar.
The spreadsheet opens.
CSV File
To download the CSV file
click File in the tool bar.
Mouse over Download as
then click Comma Separated
Values (CSV).
When download completes, click
the file to open.
You may also see the
Summary of Responses
by clicking Responses
and then Summary of Responses.
A summary like this appears.
