This letter comes as a follow up to the Notice of Performance Improvement Plan sent to you dated
_____________ indicating the specific areas where your job performance is demanded to significantly improve
on. Review of the first two performance periods yielded results as shown in the Coaching Log herein
appended. Based on such result, further improvement is strongly felt to be demonstrated in the following
areas being key business drivers for the Program:

At this point, wed like to extend a gentle reminder that performance is all there is to sustain business with the
Client as well as continued employment on your part, taking into consideration the impression we create as a
professional service provider to our Client and the pertinent Company policy on employee job performance.
Significant improvement therefore is seriously sought for in the key operating areas previously mentioned. We
hope that this is taken seriously as well from your end.
Management is open to extending all support necessary towards your achievement of a better performance
score in the next review, so please dont hesitate to approach us thru your first line supervisor (FLS) or your
program manager if there are concerns that require our attention and assistance.

ACKNOWLEDGMENT

I hereby acknowledge that I have been given formal orientation on and was made aware about the PCCWTs
Performance Management Policy even during the early phase of my employment with the Company.

I acknowledge further that after previous discussions regarding my Failed/Poor job performance, I am
hereby subjected to undergo the Performance Improvement Plan, the guidelines and processes of which are
fully and clearly explained to me by my supervisor and other company representatives.

I acknowledge furthermore that my continued failure to meet the performance targets and completely pass
set this Performance Improvement Plan may eventually result to my Dismissal from service consistent with the
Employment Contract I signed up with the Company.
