AFFIDAVIT
I, JUNE CEASAR LIM SABANGAN, SINGLE, OF LEGAL AGE, FILIPINO
CITIZEN, RESIDENT OF #015 PAGWADAN BRGY. MADAMBA DINGRAS, ILOCOS
NORTE, AFTER HAVING BEEN DULY SWORN TO IN ACCORDANCE WITH LAW,
HEREBY VOLUNTARILY DEPOSE AND SAY;
1. I am a graduate of Dingras National High School in the year of 2007 and completed the
four years in high school and received the corresponding high school scholastic cards
and good moral certificate.
2. That the above-mentioned scholastic cards and good moral certificate were kept by me
inside an envelope and put in a cabinet at our house in Dingras, Ilocos Norte.
3. That lately, as I decided to pursue my studies in college, I looked for the said documents
in the said cabinet where I placed it, I was surprised and dismayed to find out that the
scholastic records and good moral certificate were no longer there.
4. That I inquired from the members of the household if ever they have seen these
documents but they answered negatively.
5. That I exerted diligent search and inquiry into looking and locating for the abovementioned documents but to no avail and now I consider these lost and beyond recovery.
6. That I am executing this affidavit to attest to the foregoing facts and hereby request the
proper authorities for the replacement or issuance of another copies of the foregoing lost
documents.
