Goals
1) To learn at least one new thing about my assigned resident each time I attend my
clinical placement until the end of the semester.
2) To perform hand hygiene as necessary (or whenever in doubt) when practicing in
the clinical setting until the end of the semester.
Evaluation
Did I achieve my goal and what is the evidence?
1) This was not a goal that I ended up achieving completely. This however, was not
due to a lack of relational practice skills. About half way through my placement I
was moved to a different section on the floor and did not see my assigned resident
very often. This made it difficult to build a therapeutic relationship with my
assigned resident and therefore difficult to learn new things about her in order to
achieve my goal. Although this goal was not achieved this semester, I plan on
carrying it into my future placements in order to further improve upon relational
practice skills.
2) I do believe that I have achieved this goal. It has quickly become second nature
for me to perform hand hygiene in situations like before and after caring for a
resident, after performing tasks like perineal care, and before and after starting my
shifts. Additionally, my peers and other health care staff at the facility were
excellent prompts for moments to perform hand hygiene. For this reason it has not
been necessary to review incidences for hand hygiene, and I can confidently say
that I have achieved this goal.

Goals
1) To incorporate and promote confidentiality practices into my clinical practice
every day that I attend clinical placement and whenever I notice that clinical
practice (specifically its residents) become a topic of discussion outside of the
clinical setting through till the end of the semester.
2) To be considerate and aware of any unique cultural or spiritual beliefs my
assigned resident may practice throughout the entire semester.

Feedback from my resident, colleagues, other staff at the facility, and
clinical instructor.
My assigned residents chart on which any significant cultural or spiritual
practices would be indicated
External resources (e.g. Internet, RNAO BPGs) when more knowledge
about a certain culture or religion, or providing care for individuals with
certain cultural or religious beliefs would be beneficial in providing my
assigned resident with more culturally sensitive care
Timeframe: I will conduct the activities listed above each time I attend clinical practice
and anytime confidentiality of residents at the long term care facility I am assigned to
becomes an area of concern, until the end of the semester.
Evaluation
Did I achieve my goal and what is the evidence?
1) This is a goal that I am positive I have achieved. There have been situations
outside of the clinical setting where a breach in confidentiality could have
potentially happened, and I acted in ways that deterred myself and others from
this potential breach. I have personally reflected on these situations, and am proud
to say that I was able to help maintain the confidentiality of the St.Josephs
residents. Additionally, when it came to assignments and written reflections, I
always ensured that I used either initials or Patient X for identification which
also helped to maintain resident privacy and confidentiality. I plan on continuing
to carry out the success of achieving this goal into further placements and
practice.
2) This goal is one that was difficult to achieve because I was never faced with any
situations where cultural/spiritual beliefs were an issue with providing patientcentered care. Since the practice of cultural sensitivity was never really required,
this goal was never fully achieved. When I come across a situation in the future
that involves the implication of cultural awareness and sensitivity, I will revisit
this goal and my plans on how to achieve it so that I may be successful in
achieving it.

Goals
1) To ensure that I do not work outside my scope of practice throughout the semester
at my long term care NURS 1020 clinical placement.
2) To maintain an adequate level of personal fitness to better perform certain health
care practices (e.g. lifts and transfers) throughout the rest of the semester.

Supportive relationships with the facility staff, and clinical instructor,
Professional feedback from facility staff, and my clinical instructor

Timeframe: The activities listed above are ones that I will take action with each time I
attend clinical practice until the end of the semester in April 2013.
Evaluation
Did I achieve my goal and what is the evidence?
1) This is a goal that I have definitely been successful in achieving. As each week
went by and I established new sets of skills, I ensured that I implemented only the
skills that I have been taught and practiced into my clinical practice. The fact that
my clinical instructor was well aware of the new skills I had established each
week, gave me a great support system to ensure that when pressures were there to
work outside of my current scope of practice, that I could prevent and avoid it.
Anytime it may have been expected of me to lift/transfer a resident without the
help of a health care aid for example, I expressed how that was not a task I am not
able to do alone, and with the support of my clinical instructor in place, refusing
this task was never an issue. Thankfully most of the staff at the facility were also
very understanding and would usually ask if I was able to perform certain tasks.
There is not one situation that I can recall where I worked outside of my scope of
practice, and therefore I can say that I successfully achieved this goal.
2) This is another goal that I unfortunately did not achieve. I found that it has not
been easy for me to adhere to my workout schedule throughout the semester due
to a heavy workload. Therefore I have not been going for regular workouts and
have not been able to achieve this goal. Perhaps some alterations to my workout
schedule could be made so that getting some occasional exercise can be more
feasible, and I can successfully achieve this goal in the future.

Goals
1) To incorporate aspects of the Excellent Care For All Act (ECFAA) into my
clinical practice until the end of the semester.
2) To keep myself up to date on all major nursing and health care issues occurring
regionally, provincially, and nationally throughout the remainder of the semester.

Evaluation
Did I achieve my goal and what is the evidence?
1) This is another goal I believe that have achieved. Anytime I would help a resident
get ready for the day/bed, help them eat at meals, or perform assessments for my
own practice, I would ensure I had the permission of the resident before going any
further, and of course if permission was not given, I did not follow through with
the task. However certain aspects of my scope of practice were never put into
action (e.g. documentation), and therefore certain aspects of the ECFAA were not
incorporated into my clinical practice. As some of these aspects of the ECFAA
are able to be implemented into my practice, I will ensure that they area followed
so I may be able to achieve this goal.
2) This was a goal that I found very difficult to achieve. Aside from issues that were
discussed at CNSA conferences, and the occasional issue that was discussed
in/article that was read for my NURS 1002 class, I did not keep myself up to date
on regional/provincial/national nursing issues. I have not actively been reading
peer-reviewed, scholarly nursing journals. I wouldnt say that I have completely
achieved this original goal, but I have completed it to the extent that I am
currently able to. As a student it is hard to find time for reading for things other
than your courses. By getting involved with CNSA and attending their
conferences/workshops, I believe that I have made myself more aware and up to
date on regional/provincial/national nursing issues than I probably would have
been if I didnt get involved. After graduation, I believe it will be more likely that
I will be able to reading nursing journals as a way to maintain nursing/health care
awareness.

1) To participate in the CNOs Quality Assurance (QA) program throughout my
experience in clinical practice until the end of the semester.
2) To ensure that I always have a clean, wrinkle free pair of scrubs to wear to
clinical practice throughout the remainder of the semester.

1) This goal is one that I also believe I have successfully achieved. Although I have
not documented feedback I have received throughout the semester, I have
