PORTFOLIO ITEMS: Create your online portfolio using weebly.com (a free site) or similar site as
your instructor permits. Save a copy of this page as a word document as signatures and materials
are added. Student should provide a brief title and the date in the appropriate box when
adding/deleting an item from the portfolio. Faculty teaching each course will sign in their
appropriate box and also place initials in the box next to the students written description. (If
online, use electronic code for signature when returning to student.) The colored boxes represent
the required minimum number of items for each criterion.
