Children Health Services
CC4C

CC4C is a program offered at no


charge for children birth to 5 years of
age who:
Have long term medical conditions;
and/or
Are dealing with challenging levels of
stress; and/or
Are referred by the childs doctor

When the Care Manager receives a


referral they engage the client with
CC4C and determine whether the
child and family can benefit from
CC4C services.
CC4C care managers work with
families through:
Home visits
Phone calls
Visit the doctor with client
Other types of contacts

Care
Coordination
for Children
(CC4C)
The goal of CC4C is to improve
your childs care by linking you to
services that will meet the specific
needs of your child and family
Requires a BSW to be a CC4C
Care Manager

Referral criteria
Children with Special Health Care Needs (chronic
physical, developmental, behavioral or emotional
conditions) who require health and related services of a
type and amount beyond that required by children
generally.

Children exposed to severe stress in early childhood,


including:-- Extreme poverty in conjunction with
continuous family chaos
-- Recurrent physical or emotional abuse
-- Chronic neglect

-- Severe and enduring maternal depression


-- Persistent parental substance abuse
-- Repeated exposure to violence in the community or
within the family
Children in foster care who need to be linked to a Medical
Home
Children in neonatal intensive care needing help
transitioning to community/Medical Home care.

CC4C Services
Services provided by CC4C
care managers are tailored to
patient needs and risk
stratification guidelines. A
comprehensive health
assessment, including the Life
Skills Progression, assists the
care manager in identifying a
childs needs, plan of care and
frequency of contacts required.

Coordination for Children Program at the
Granville Vance District Health

call of duty to help children and families
get healthcare in Vance and Granville
counties. Massey's philosophy of working
with others toward a common vision
shows in her relationships with other
public health staff and management. She
completes home visits with ease in a
challenging environment, and she serves
as a role model for her colleagues in the
Health Department and in other partner
agencies.

GVDHD
Practice Management
This is a new system that is being implemented in
GVDHD to help generate more revenue by reducing wait
times and moving in the direction of Open Access. As
of now GCDHD has Mondays and Fridays are
dedicated to Clinic hours, Tuesdays are maternal
health, Wednesdays are Childrens Health, and
Thursdays are Family Planning. This set up allows
patients to be seen for these services on these certain
days but with open access the patience can access
them easier and when they need them. This is a need
and the future direction of GVDHD!
