Question:
Why are there duplicate medications when I am completing the Admission Meds Reconciliation and how do I manage these?
1. If a Provider enters a medication order on admission, and the RN documents the same medication as a
home med, the medication will appear as a duplicate, one as an inpatient med and one as a home med.

2. If the home medication and inpatient hospital medication doses are identical, Do Not Continue the
home medication
Note: Home Med is in a Suspended status,
5. If there is a duplicate inpatient medication, right click on duplicate order and select Void.
Note: Voided duplicate medication will no longer appear on the Meds Reconciliation.
3. This will ensure that the medication will display correctly on the Meds Reconciliation and the patients printed
Discharge Instructions.
4. If the home medication and hospital medication doses are different, and you want to continue the
hospital medication at home, select Continue After Discharge for the hospital medication.
5. To reconcile the home medication, select Do Not Continue After Discharge.
Question:
I want to discharge my patient on an increased/decreased dose, different route, or different
frequency of the same medication that was ordered in the hospital and/or taken at home.
How do I reconcile the current medications and create a new prescription?

3. Click on the new prescription to view details box, and select appropriate order details.
Note: Change Dose field in details box to See Instructions and enter taper in Special Instructions.

3. To avoid a medication populating under the Medications to Continue with no Changes category,
choose to continue either the home medication OR the inpatient medication.
Question:
Why do some medications fall under the category of Contact Your Physician Prior to Taking the
Following Medication on the Discharge Instructions?
1. This will occur if an Admission Meds Reconciliation or Transfer Meds Reconciliation is done or modified
AFTER user has completed the Discharge Meds Reconciliation. This will also occur if user leave a medication
un-reconciled.
2. All medications with this statement will need to be reconciled again. To do this select Create New Rx, if
patient should continue taking this medication at home or do Not Continue After Discharge.
NOTE: Discharge Instructions should ALWAYS be reviewed from the Depart button. If the medication section
does not appear correctly for the patient, go back to the Discharge Meds Reconciliation and revise.
