Meds Reconciliation FAQs for Providers

Admission Meds Reconciliation


Question:
When I try to complete my Admission Meds Rec, why do I get a pop-up message stating there?
1. If this pop-up appears, the documentation of
the patients medication history has not been
Completed, and you can not move forward
with the Admission Med Rec process.
2. Message will no longer appear after Nursing
or the Provider has documented the Med Hx.

Question:
Why are there duplicate medications when I am completing the Admission Meds Reconciliation and how do I manage these?
1. If a Provider enters a medication order on admission, and the RN documents the same medication as a
home med, the medication will appear as a duplicate, one as an inpatient med and one as a home med.

2. If the home medication and inpatient hospital medication doses are identical, Do Not Continue the
home medication
Note: Home Med is in a Suspended status,

3. If the home medication and hospital medication doses are different, and you want to continue the inpatient hospital medication , choose Do Not Continue for the home medication.
4. The home medication will appear in a Suspended status, and the inpatient medication will continue as
Ordered.

5. If there is a duplicate inpatient medication, right click on duplicate order and select Void.

Note: Voided duplicate medication will no longer appear on the Meds Reconciliation.

Discharge Meds Reconciliation
Question:
Why are there duplicate medications when I am completing the Discharge Meds Reconciliation?
1. If a home medication has been converted to an Inpatient medication, you will see the med listed twice on
the Discharge Meds Reconciliation.

2. If the home medication and inpatient hospital medication doses are identical, ALWAYS continue the home
Medications.

3. This will ensure that the medication will display correctly on the Meds Reconciliation and the patients printed
Discharge Instructions.

4. If the home medication and hospital medication doses are different, and you want to continue the
hospital medication at home, select Continue After Discharge for the hospital medication.
5. To reconcile the home medication, select Do Not Continue After Discharge.

6. This will ensure that the medication will display correctly on the Meds Reconciliation and the patients printed
Discharge Instructions.

Question:
I want to discharge my patient on an increased/decreased dose, different route, or different
frequency of the same medication that was ordered in the hospital and/or taken at home.
How do I reconcile the current medications and create a new prescription?

1. Select Create New Rx for the home medication


Note: If medication was given as an inpatient in the hospital the radio dial will automatically default to
Do Not Continue After Discharge.
2. Click on the new prescription to view details box, and select appropriate order details.
3. Select down arrow next to the medication name to view alternate dose options.

5. This will ensure that the medication will display correctly on the Meds Reconciliation and the patients printed
Discharge Instructions.

Question:
How do I modify a tapered medication dose at discharge?
1. Select Do Not Continue After Discharge for the home medication.
NOTE: NEVER select Create New RX for any listed home medications.

2. Select Create New Rx for the inpatient hospital medication.

3. Click on the new prescription to view details box, and select appropriate order details.
Note: Change Dose field in details box to See Instructions and enter taper in Special Instructions.

4. This will ensure that the medication will display correctly on the Meds Reconciliation and the patients
Printed Discharge Instructions.

Question:
Why do duplicate medications fall under the category of Medications to Continue with no
Changes on the Discharge Instructions?
1. Medications will be placed under this category if both the home medication and the inpatient hospital
medication were continued on the Discharge Meds Reconciliation.
2. If both are continued, the patients Discharge Instructions will display incorrectly.

3. To avoid a medication populating under the Medications to Continue with no Changes category,
choose to continue either the home medication OR the inpatient medication.

Question:
Why do some medications fall under the category of Contact Your Physician Prior to Taking the
Following Medication on the Discharge Instructions?
1. This will occur if an Admission Meds Reconciliation or Transfer Meds Reconciliation is done or modified
AFTER user has completed the Discharge Meds Reconciliation. This will also occur if user leave a medication
un-reconciled.
2. All medications with this statement will need to be reconciled again. To do this select Create New Rx, if
patient should continue taking this medication at home or do Not Continue After Discharge.

NOTE: Discharge Instructions should ALWAYS be reviewed from the Depart button. If the medication section
does not appear correctly for the patient, go back to the Discharge Meds Reconciliation and revise.

Question:
Can I change the Duration field to something other than what appears in the drop down
menu?
1. From the details box, place cursor into Duration field to view drop down menu.
2. Type desired number into the Duration field.
3. Select Show All from the drop down menu.

Additional options will display from the drop down menu in the Duration field.
4. Select from day(s), minute(s), month(s), second(s), time(s), week(s), or year(s).

Customized duration will now appear in the details box.

Question:
How can I continue an IV antibiotic on the Discharge Medication Reconciliation if the action
fields are not available?

1. Click Add button from top left of Order Reconciliation box.


Add Order box appears.
2. Begin Typing name of IV antibiotic in Find field and select
appropriate product.
3. Click Done.
IV Antibiotic appears as a new prescription under Home Medications heading.
4. Complete details for IV antibiotic Note: If Duration field is completed, selected duration will appear on the
patients Discharge Instructions.
