Client Evaluation

Description
Following the presentation of a case,
student went through process of
evaluating and considering all client
factors, creating an intervention plan,
and creating goals for overall treatment.
ACOTE Standards:
1: Evaluated clients occupational
performance based on case and
used appropriate assessment
tools to assess the clients needs,
contextual factors, etc.
2: Used evaluation findings,
theory and practice models to
develop intervention plan and
strategies for treatment.
Student conducted a needs analysis at a
community setting that would benefit
from occupational-based services.
Student created a proposal for a
occupation-based program at the site
including research to support.
ACOTE Standards:
6: Demonstrated ability to plan,
develop, organize, and market the
delivery of services. Assessed the
determination of needs for
services and formulation of
staffing and management.
7: Effectively located, evaluated,
and demonstrated understanding
information and quality of
evidence.
Student conducted an evaluation of
client for neuro-rehab treatments. From
evaluation findings, student created
goals and treatment plan for client.

Student prepared discharge report for
client. Client was seen over the course of
three months. The discharge contains
summary of intervention and progress
toward goals, outcomes, and future
recommendations for client, family, and
possible other healthcare providers.

3: Created a plan for discharge in
collaboration with the client.
Needs of the client were reviewed
with the client and significant
other as well as
recommendations and clients
progress towards goals.
5: Effectively interacted through
written, oral, and nonverbal
communication with client in a
professional manner that clarifies
responsibility in care plan.
Student reflected on an experience from
Level II fieldwork that placed them in an
ethical dilemma. Student reflected using
four strands of reflection to analyze
situation.

analyzing issues and making
decisions to resolve ethical
conflicts.
Student completed written
documentation for client that was seen
by student in a Level I fieldwork setting.
Documentation provides information of
session for billing and other staff as well
as proper evaluations for
reimbursement.

4: Documented occupational
therapy services to ensure
accountability of service and
meet standards for
reimbursement at the site.
Student worked in group to review
evidence in order to create proposal for
research topic as headed by staff
