1. That I am the sister of Ma. Rhoda A. Magaling;
2. That Ma. Rhoda A. Magaling is a resident of the above address.
3. That the date of birth Ma. Rhoda A. Magaling is May 18, 1968
4. That the date of birth of Ma. Rhoda A. Magaling is wrongly mentioned in her
Marriage Certificate
5. That the affiant wants to get corrected the date of birth and address of her
sister Ma. RhodaA. Magaling to her Marriage Certificate as May 18, 1968 instead
of May 18, 1969 and resident of San Juan City instead of Taytay, Antipolo Rizal
6. That I am executing this affidavit to attest to the truth of the foregoing facts and
for whatever legal purpose it may serve.
