Make sure you have the latest application form and deadline information; programs may
have been revised. Application material is dated by year; for example, if you are applying
in 2014, you must use a 2014 application.
Applications submitted in French to programs outside the Franco-Ontarian Arts Office are
translated into English by accredited translators for assessment. By request, these
translations are available to applicants for information only, after results have been
announced. Works of art (e.g. film scripts) or written materials that have not been asked
for in the application are not translated.
In programs outside the Franco-Ontarian Arts Office, advisory panels and juries are held
in English. If there are French-language applications in those competitions, at least one
advisor/juror is francophone. If a competition receives enough French-language
applications, a separate meeting with francophone advisors/jurors is convened.
You must submit your application to the OAC on paper. OAC forms may be filled in on
screen by computer then printed, or the forms may be filled in by typewriter or legible
handwriting in black ink. Other written materials, such as the project description or artist
statement, must be computer-generated in a type size no smaller than 11-point or
typewritten and submitted on 8" 11" white paper.
Please provide the required number of copies of the application material. We keep one
set for our files and send the other copies to the advisory panel/jury for assessment.
These copies must be on 8" 11" white paper and can be double-sided copies.
We will not accept applications that have been faxed, emailed or sent on a disc.
Incomplete applications will go forward to the advisory panel/jury as is. We only follow up
on missing elements of the application with first time applicants to the OAC. We do not
accept any additional materials after the deadline.
Make sure your application is complete, signed, accurate and readable, that sections are
arranged in the same order as on the checklist, and that the package is submitted with
the correct number of copies. These requirements are your responsibility.
We will not notify you automatically when we have received your application package.
If you want us to confirm your application has been received, please enclose a selfaddressed, stamped postcard.
ii
Get your application in on time. Late applications are ineligible and will be returned to
you by mail at any point in the process after the deadline date.
You may also hand deliver your application to our office on the 5th floor of 151 Bloor
Street West, Toronto, up to 5 p.m. on the deadline day.
After 5 p.m. on the deadline day ONLY, you may leave your application packages with
building security until 8 p.m. Your application must be complete (photocopied and sorted)
before you drop it off.
iii
iv
If you wish to use technologies with which you are unfamiliar, you may work with the assistance
of a specialist or mentor. Please include letters confirming this type of assistance.
If your work requires another form of permission, such as community recognition or elder or
master artist approval, provide letters of agreement or a statement that indicates how you will
acquire permission to undertake your project.
The required number of copies must be included with your application. Facsimile, electronic scan
or email copies of letters of agreement are acceptable only if submitted with your application.
The letters will not be returned to you. Original signatures may be requested by the OAC Media
Arts Office at any time.
If you submit your letter(s) separately from the application, either prior to or after the deadline
date, the letter(s) will not be forwarded to the jury.
INSTRUCTIONS:
After completing your application, sort the copies into 6 individual sets.
Each set should contain all items in the same order as they appear on the checklist
above. Please number the pages.
Send the entire application package to the program name and address on the cover
page of this application.
our responsibility to help correct systemic barriers and historical challenges faced by
Aboriginal peoples and people of colour;
our role in supporting the arts as a critical link between language and culture for
francophone people;
the need to support new generations of artists (18 to 30 years old) in order to seed
growth in all our arts communities.
We are gathering information to find out how well OAC grants serve Aboriginal artists, artists of
colour, francophone artists and new generation artists, and organizations that support the work
of artists from these communities.
This information will not be used to assess your application, will not be passed on to the
assessors and will not be attached to your file. After we prepare a summary report using these
statistics, this form will be destroyed.
Your response to the questions in this section is voluntary.
OACs definition of the term Aboriginal means Status and Non-Status, Mtis and Inuit people.
OACs definition of the term Francophone is a person who learned French at home and still
understands it.
The term person of colour is based on the government of Canadas definition of visible
minorities, which is defined as persons, other than Aboriginal peoples, who are non-Caucasian
in race or non-white in colour.
OACs definition of the term new generation is a person who is 18 to 30 years of age.

BUDGET INSTRUCTIONS
Only fill in the Application column. If you are awarded a grant, you will be required to fill in the
Confirmed column once all sources of revenues are confirmed and eventually you will submit
the Actuals as part of your final report.
Attach a detailed budget of no more than 10 pages. Provide additional Budget Notes if
appropriate.
If there are corporate sponsors involved, attach copies of contracts or letters of agreement. This
program does not fund television projects, so broadcast sales and licences are not eligible
sources of revenue.
The total amount of expenses must match/balance with the total amount of revenues. Indicate
from where you are planning to derive revenue in addition to your request to the OAC, even if
that revenue is pending. Indicate whether those sources of revenue are confirmed [C] or
pending [P].
If you are applying for a work-in-progress your budget should include all expenses already
incurred and the sources of revenue that offset them.
Crowdsourcing campaigns should be completed prior to application so you can state the amount
of confirmed revenue from these sources.
Provide documentation of deferrals and donations.
Important: If you are applying to other government agencies, organizations or companies for
funding or sponsorship of the same project, you must declare the full amount that you are
requesting from each source in the revenue section of your budget. You must submit to the OAC
the same budget that has been sent to all other funders and sponsors.
Your projects total budget cannot exceed $75,000 (including donations and deferrals).
Expenses may include:
costs of promoting, reproducing and distributing your work, to a maximum of 10% of the
grant amount
Installation send audio-visual material from 1 previous installation. You may also
send 2 images illustrating the installation on-site.
Exception: If you are seeking a grant to complete a work that is already in progress, you are
encouraged to submit at least a first assembly of the work-in-progress (film), or a rough edit
(video) or documentation of the work to date (installation, web, game, etc.) with your
application, as one piece of support material. Do not submit raw footage. You must also
submit another piece of support material that is a previously completed work.
Student work: Work created as part of the curriculum of an educational institution, or using
the resources of an educational institution, is not eligible. Work that was created while you
were a student, but independent of the educational institution, is considered independent
work, not student work. If appropriate, please provide a note of explanation.
If you have any questions about the eligibility of your support material, please contact
the Program Officer prior to the deadline.
All support material must be included in the application package. There will be no extensions
for the receipt of support material.
Jurors have limited time to view support material (usually a total of 5 minutes), so provide
directions to the DVD section that you feel is most relevant to your current application.
Make note of any elements in your support material that are of special relevance to your
proposed project.
Do not send excerpts or demo reels. Jurors must have the option to view a work in its
entirety.
