REBATE RULES
Qualified independent repair facilities (IRF) and body shops are
entitled to the rebates indicated on this form. Excludes Fleet
accounts, auto auctions and mobile repair units. Non-powertrain
assembly rebate offers are limited to up to six (6) rebates per IRF/
Body Shop for each product offering during promotion dates of
1/01/15 and 4/30/15 to be eligible for the rebate (unless otherwise
noted). Where applicable, multiple quantities must be purchased on
same invoice (or line item) to qualify for rebate. Purchase invoice and
rebate form are required at the time of submission. The repair order
may be requested and required in order to process the claim. Rebate
must be submitted by IRF/Body Shop. Visit http://ford.partsrebates.
com for complete list of eligible program part numbers and additional
unlimited powertrain offers. Rebate form must be completed in full
and submitted online or postmarked by May 31, 2015 along with
supporting documentation. Rebate form must be complete and
legible in order to be accepted. Product must be purchased from a
Ford or Lincoln Dealership, a Ford Authorized Distributor or an auto
parts store. Non-Ford Automotive OE Dealerships are eligible if they
do not share ownership (private, public, whole, part) with a Ford
or Lincoln Dealership. Open to legal residents of U.S. only. All Ford
and Lincoln Dealers and employees are ineligible. Purchase invoice
date and sale date must fall within program period. Handwritten
purchase invoices will not be accepted. Retain a copy of rebate and
original invoice for your records in the event backup documentation
is requested. Rebate check will be issued 6 to 8 weeks from receipt
and validation of rebate and proof of purchase. Check will be payable
to the business name. Rebate void if purchased, sold or bartered
for cash or altered in any way. Rebate form may be reproduced
for multiple uses. Not valid in conjunction with any other offer.
