NOTICE TO EXPLAIN/PREVENTIVE SUSPENSION*

It was reported to us/ It has come to our attention that: (Give a more
or less detailed narration of what happened).
These alleged acts of dishonesty are considered as: (you can choose
more than one, depending on the violation)
a) Serious misconduct;
b) Willful disobedience;
c) Gross and habitual neglect of duties;
d) Fraud;
e) Willful breach of trust;
which are grounds for dismissal for employment under Article 282 of
the Labor Code.
Accordingly, please explain within 5 days upon receipt of this notice
why no disciplinary action including dismissal, should be taken against you
for having committed the said offense.
Failure to submit a written explanation within 5 days from receipt of
this notice will be considered as a waiver of your right to be heard and shall
be construed as an admission on your part of the commission of the abovestated offenses.
*Furthermore, you have been placed under a 30-day Preventive
Suspension starting on the date of receipt of this notice, pending
investigation of the above-mentioned charges against you. (Optional if the
offense involves serious matters like theft or physical violence).
