HSCI 495 FIELD EXPERIENCE PLAN

Hours and Days Student Will Be At the Site: Mondays, Wednesdays, from 11am-5pm,
DESCRIPTION OF PROJECT TO BE COMPLETED
1. The Health Education Center aims to promote healthy living styles among the San
Bernardino community by offering a variety of nutritional and health classes. The
main goal of the Health Education Center is to empower the community and help the
community prevent and treat nutrition-related health problems. The health education
classes include stress management classes, diabetes workshops, and weight
management classes among others. In addition, the center offers childbirth
education and prepares women for the breastfeeding process. The role of the
preceptor is to prepare and further capacitate the student in the field of health
education. The preceptor will offer the student the opportunity to further learn and
acquire knowledge, skills, and experiences involving the CHES competencies. The
role of the student is to learn from the preceptor and utilize the tools given to grow in
the field. The student will complete the tasks given by the preceptor by the date
indicated above. Some of the tasks include but are not limited to research, data
entry, attend health classes, and to prepare nutritional health related classes.
2. State the measurable objectives and indicate the related program learning
outcome for the Field Experience that will be accomplished:
1. By January 23, 2015, the student will research San Bernardino County records
for gestational diabetes. The intern will report the findings to the health instructor
at the Community Hospital of San Bernardino Health Education Center and will
create a chart to organize the findings on gestational diabetes. (LO 1.2; LO 9.2)
2. By February 6, 2015, the intern will create a lesson plan to inform pregnant
women about nutrition during their pregnancy. An appealing title for the lesson
plan will be created and will be reported to the health instructor for approval. (LO
5.3; LO 8.1)
3. By February 13, 2015, the intern will create a flyer to promote the nutrition class
for pregnant women. The flyer will have the important information regarding the
presentation such as time, place, date, and brief overview. (LO 9.1)
4. By February 20, 2015, the lesson plan will have been completed and will be
presented to the health instructor to receive approval. In addition, the intern will
have attended two community meetings in San Bernardino to promote the
nutrition class for pregnant women (LO 5.1)
5. By March 6, 2015, the class will be presented at the Health Education Center in
San Bernardino to the community. (LO 1.3; LO 13.1)
3. List the tasks that the student will complete to accomplish the objectives:
a. Research county data to determine the statistics that will used
c. Enter data in a format in which it will be easy for the community to comprehend
b. Meet with and inform the preceptor on the progress of the nutrition class
d. Attend prenatal and childbirth classes to be informed on the topics needed to be
covered.

e. Implement creative techniques to enhance the attention of the community.
