I built a particularly strong relationship with my


resident and I have been to make her feel at ease
even with her severe dementia. I always introduced
myself and asked her questions that were simple for
her to understand. Effectively using the techniques
learned in our NURS 1002, I communicated in the
most effective manner without belittling their
dignity. While feeding her, I was able to assess how
she responds and evaluate how to proceed with the
process.
While reading over residents charts, I was able to
see their preferences of how to provide daily care.
For example, I knew one resident preferred to watch
a certain show which happened to fall during
breakfast one morning so I made sure to have it on
while I fed her breakfast. I also helped residents
with all of their activities of daily living such as
dressing, feeding, washing, and ambulating.
Over the course of my placement, I have had the
opportunities to measure residents blood pressure,
perform a very basic mental assessment on those
with severe dementia, and constantly assess for signs
of distress. I was able to identify the stage of
pressure ulcers prevalent to the residents in a longterm care facility

When providing care, I always assure to take the


residents preferences into account. For example,
while feeding, I always ask what they would like to
eat or start with and in what manner they would like

to be fed so they feel they do have some control


despite having to depend on me. Answering the call
bell is another important aspect as the resident is
seeking help even when it is hard to ask others
sometimes; this allows me to prevent the risk of any
fall or distress of the resident.
I learned during my placement of the structure of a
typical day at a long-term care facility. This made me
feel more prepared to provide safe, client-centered
care relevant to the older adult population.

at the level appropriate for a year one nursing
student

While assisting with activities of daily living, I took


all measures to maintain the safety of the residents
and preserve their dignity. For example, I would
remove all tripping hazards if the client is
ambulatory or closing the clients door when the
nurse is administering a suppository.

Writing weekly reflections after clinical


shifts has truly allowed me to examine
both my strong and weak points as a
developing nurse. I was able to receive
feedback from my instrocutor on how to
improve which was greatly beneficial to
feeling more comfortable. This has
allowed me to develop the skills I need to
acquire in this life-long learning profession
as to be able to provide the best care I

My pre-conceived notions about the older adult
population have certainly been changed since
entering the long-term care setting. All of these
residents had a life and a story before they became
unable to live independently and should be treated
with the utmost respect. Those who are in a longterm cre home are there because they need help and I
did my best to provide the best, client-centered care I
possibly could.
While completing the weekly care cards and my
medication card presentation, I have built a certain
basis of knowledge regarding chronic disease and
appropriate nursing interventions. I may not know
every single drug or disease, but I have the amount
of knowledge appropriate for a first year nursing
student and I will continue to grow and learn
throughout my schooling, and later career.
