5K and Kids K Fun Runs

LOCATION:

Adams Elementary; 2200 East 2500 North, Layton, Utah

Race will begin and end at Adams Elementary (same course as last year, maps available in the office)

SCHEDULE:

5:00 pm Late registration

6:00 pm Kids K
6:15 pm 5K

ENTRY FEE:

DONATION:

$12 per person

$10 per person (if you are registering 4 or more family members)
Our family will participate by making a donation. Donation amount $_________
Registration is due by Friday, April 17th. We encourage you to turn in your entry form EARLY!
Make checks payable to Adams Elementary.

T-SHIRT:

The entry fee includes a race day t-shirt.* The t-shirts this year are a great turquoise blue with black
writing!
*Late registrations will be accepted but you will NOT receive a t-shirt. We need to
have our t-shirt order in on Friday, April 17th.

AWARDS:

Medals for top 3 male and female runners in Kids K. Awards to top 3 male, female and 12 & under
male and female 5k finishers.

The undersigned Participant understands and is aware of the risks and hazards associated with running races. I represent that Im physically fit and capable of running in this
Event and further represent that if I have any question regarding my fitness for the event, Ive consulted with a physician. I assume all risks associated with my participation in the
event, even if those risks are caused by the negligence of someone else. I discharge and release forever Adams Elementary PTA and any other sponsors or organizers together
with their respective offices, agents, employees and other representatives of any liability whatsoever for any claim for damage, injury or death that may happen during my
participation in the event. I understand that there is no obligation to provide me with medical care as a result of my participation in the event, but that if such emergency care is
provided, all of the terms of this Agreement shall apply. The organizers of this event may use my likeness, records, or photograph for any purpose without compensation to me. I
also understand that the registration fee is non-refundable
