Drug Classes
• Antibacterial
• Urinary tract anti- infective

Therapeutic Actions
• Bactericidal; interferes with bacterial
cell wall synthesis, blocks
adherence of bacteria to
ureopithelial cells.

Indication
• Uncomplicated UTIs in woman
caused by susceptible strains of
Escherichia coli and Enterococcus
faecalis; not indicated for the
treatment of pyelonephritis or
perinephric abscess.

Contraindications and Cautions


• Contraindicated with allergy to
fosfomycin.
• Use cautiously with pregnancy,
lactation.

ADULT
1 packet dissolved in water PO.

Absorption: Rapidly absorbed
from GI
tract
Peak urine concentration: 2 – 4 hr
Elimination: Primarily

Adverse Effects

CNS: Headache, dizziness, back


pain, asthenia
GI: Nausea, abnormal cramps,
dyspepsia, diarrhea
GU: Vaginitis, dysmenorrheal

NURSING CONSIDERATIONS

Assessment

• History: Allergy to fosfomycin,


pregnancy, lactation.
• Physical: Skin color, lesions;
orientation, reflexes; urine for
analysis.

Interventions
• Arrange for culture and sensitivity test.
• Administer drug with food if GI upset
occurs.
• Monitor clinical response; if no
improvement is seen or a relapse
occurs, send urine for repeat culture
and sensitivity tests.
• Encourage patient to observe other
measures to decrease risk of UTI.
Fosfomycin Tromethamine

Teaching Points
• This drug is meant to be one-dose treatment for
urinary tract infection; improvement should be seen
in 2-3 days. If no improvement occurs, consult your
health care provider.
• Take drug with food if GI upset occurs. Do not take in
the dry form; mix a single dose packet in 90 – 120
ml of water and stir to dissolve; do not use hot
water. Drink immediately after mixing.
• You may experience these side effects: Nausea,
abdominal pain; dizziness.
• Report rash, visual changes, sever GI problem,
weakness, tremors, worsening of urinary tract
symptoms.
