Step 1. Open Chrome, Firefox, Safari, etc.

Step 2. Type ualr.edu/slufy in the search box.

Step 4. Fill-in the application completely.

Students must be completing Kindergarten to apply.

3. SLUFY Options
T-shirt size, carpool, snacks, etc.

4. Parent/Guardian Information
Use the email that is checked most often.

5. Course Selection
Rank the students top four course choices. Kindergarten
students will rank three choices.

6. Emergency/Health Information
Give at least two contact numbers for the emergency
contact.

7. Electronic Signature Statements

Read and check to agree. When checked the paragraph will
go bold.

Step 5. Click Submit

Step 6. Print the confirmation page and check

for errors.

Step 7. Click the payment form link at the

bottom of the page.

Step 8. Print out or save the Payment Form.

Complete it and return it with the $25 deposit.
