PLAAFP Statement

Kaylen is currently in 6th grade. He is proficient in single and double digit addition and
subtraction. He shows good progress in base ten numerations, and place value. Kaylens baseline
scores are as follows: Math Measurement 11/16 (68%), Math Numbers and Operations 5/16
(31%), and Math Numbers and Operations and Algebra11/16 (68%). According to his Easy
Math CBM measures given at the 2nd grade level, Kaylen is inconsistent when calculating units
of measure and time. He also has difficulty determining the value of mixed coins and selecting
efficient methods to solve for differences. Because Kaylen struggles in these areas, it is difficult
for him to keep up with math expectations for 6th grade level general education curriculum.
Continuing in the special education math group setting will be beneficial to Kaylen.
IEP Goals
At the end of the school year, Kaylen will score 57% when given Math Number and Operations
problems representing the scope of 2nd grade. He will correctly add and subtract using a number
line or a hundreds chart on 2 /3 trials as measured by Easy Math CBM.
At the end of the school year, Kaylen will score 94% when given Math Measurement problems
representing the scope of 2nd grade. He will correctly choose appropriate measurement units and
compare them to objects on 2 /3 trials as measured by Easy Math CBM.
At the end of the school year, Kaylen will score 94%when given Math Number and Operations
and Algebra problems representing the scope of 2nd grade. Kaylen will be able to correctly
determine the value of mixed numbers of coins on 2 /3 trials as measured by Easy Math CBM.

PLAAFP Statement
Brinna is currently in 6th grade. She is proficient in single and double addition and subtraction.
She shows good progress in analyzing and transforming 2 dimensional shapes, representing
common fractions, and whole number relationships. Brinnas baseline scores are as follows: x
Math Operations and Algebra, 10/16 (63%), Math Numbers and Operations, 15/16 (94%), and
Geometry, 12/16 (75%). According to her Math Easy CBM given at the 3rd grade level, Brinna
is inconsistent when representing situations using models of multiplication and division. She also
has difficulty applying sophisticated strategies based on number properties. Because Brinna
struggles in these areas, it is difficult for her to keep up with math expectations at the 5th grade
level general education curriculum. It would be beneficial for Brinna to continue in the special
education math group setting.
IEP Goals
By the end of the school year, Brinna will score 82% when given a Math Number and Operations
and Algebra assessment representing the scope of 3rd grade. Brinna will correctly relate
multiplication and division as inverse operations on 2/3 trials as measured by Easy Math CBM.
By the end of the school year, Brinna will score 100% when given a Math Number and
Operations assessment representing the scope of 3rd grade. Brinna will correctly compare and
order fractions on 2/3 trials as measured by Easy Math CBM.
By the end of the school year, Brinna will score 88% when given a Geometry assessment
representing the scope of 3rd grade. Brinna will be able to correctly build, draw, and analyze two
dimensional shapes on 2/3 trials as measured by Easy Math CBM.
