AFFIDAVIT OF SUPPORT
I, JULIUS ROY ALVIOLA, of legal age, single, Filipino and a resident
of Bonifacio Extension, Davao City, under oath depose and say:
1. I am a duly registered taxpayer of the Bureau of Internal
Revenue with Taxpayer Identification No. 426-028-410;
2. I am applying for Additional Exemptions as provided for by our
present tax laws and revenue regulations, as Qualified
Dependent Children, my three (3) daughters, namely:
a. JONALAINE ALMAZAN ALVIOLA born on August 2, 1997;
b. JORNALAINE ALMAZAN ALVIOLA born on December 15,
1998;
c. JULIANA ALMAZAN ALVIOLA born on September 23, 2001;
3. That I have provided and continuing to provide my three (3)
daughters, namely JONALAINE ALMAZAN ALVIOLA, JORNALAINE
ALMAZAN ALVIOLA, JULIANA ALMAZAN ALVIOLA with SUPPORT as
defined by our laws, in compliance with my parental obligation
as their father;
4. That I am executing this affidavit to the truth of the facts and for
such other legal purposes this affidavit may serve.
