Student Activity Information Form
Directions:
This is a survey form to gather information.
Any and all information can be used by the faculty council to assist with
the selection process.
Please complete all sections.
Do not be modest.
Completion of this form does not guarantee selection.
Please refer to the accompanying sheets and the Student Handbook for
explanation and definitions of the selection process.
Please type or print in black ink.
Do not use the back of any page.

I Administrative Information:

II Co-curricular Activities: List all activities in which you have participated in
school. Include clubs, teams, musical groups, etc. Also please include the years
you participated and your major accomplishments in each.

III Leadership Positions: List all elected or appointed leadership positions held in
school or community activities and the year you held the position. Only those
positions in which you were directly responsible for directing or motivating others
should be included. For example, class or club officer; committee chairperson;
team captain; newspaper editor; or community leader.

IV Community Activities: List community activities and the year in which you
have participated. Please note any major accomplishment in each. These
should be any activities outside of school in which you participated for the
betterment of your community. For example: Boy/Girl Scouts, volunteer groups,
4-H, church group, or community art endeavors.

V Recognition and Awards: List below any honors or awards that you have
received which support your bid to be selected for membership in the National
Junior Honor Society.

VI Essay: On a separate sheet of paper, write a five paragraph essay on what it


means to be a good citizen.

National Junior Honor Society Selection Procedure


Students may not apply for membership to the National Junior Honor Society
(NJHS). Membership is granted only to those seventh and eighth grade students
selected by the faculty council.
Students academic records are reviewed to determine scholastic eligibility.
Students must have a 3.2 grade point average out of a possible 4.0 in their five
major subjects to be scholastically eligible for the NJHS. The following lists the
grade point equivalents for each letter grade: A-4, B-3, C-2, D-1, and F-0.
Scholastic eligibility for seventh graders is based on the grade point average
attained in the first three quarters of the seventh grade. For eighth graders
scholastic eligibility is based on the grade point average attained in the seventh
grade and the first three quarters of the eighth grade.
Students who are scholastically eligible are notified and told that for further
consideration they must complete the Student Activity Information Form.
The completed Student Activity Information Form is reviewed by the faculty
council and additional comments are sought from other non-voting members of
the faculty.
The faculty council uses the following guidelines to assess leadership, service,
character, and citizenship.
Leadership
The student who exercises leadership:
Is resourceful in proposing new problems, applying principles, and making
suggestions.
Demonstrates leadership in promoting school activities.
Exercises influence on peers in upholding school ideals.
Contributes ideas that improve the civic life of the school.
Is able to delegate responsibilities.
Exemplifies positive attitudes.
Inspires positive behavior in others.
Demonstrates academic initiative.
Successfully holds school offices or positions of responsibility, conducts
business efficiently and effectively, and is reliable and dependable without
prodding.
Demonstrates leadership in the classroom and in school activities.
Is thoroughly dependable in any responsibility accepted.

Service
The student who serves:

Is willing to uphold scholarship and maintain a loyal school attitude.


Participates in some outside activity: Girl Scouts, Boy Scouts, church
groups, volunteer services for the aged, poor, or disadvantaged, family
duties.
Volunteer dependable and well-organized assistance, is gladly available,
and is willing to sacrifice to offer assistance.
Works well with others and is willing to take on difficult or inconspicuous
responsibilities.
Cheerfully and enthusiastically renders any requested service to the
school.
Is willing to represent the class or school in inter-class and inter-scholastic
competition.
Does committee and staff work uncomplainingly.
Shows courtesy by assisting visitors, teachers, and students.

Character
The student of character:

Takes criticism willingly and accepts recommendations graciously.


Constantly exemplifies desirable qualities of personality (cheerfulness,
friendliness, poise, and stability).
Upholds principles of morality and ethics.
Cooperates by complying with school regulations concerning property,
programs, office, halls, etc.
Has powers of concentration and sustained attention as shown by
perseverance and application to studies.
Demonstrates the highest standards of honesty and reliability.
Shows courtesy, concern, and respect for others.
Observes instructions, rules, and punctuality, both inside and outside of
the classroom.
Manifests truthfulness in acknowledging obedience to rules, avoiding
cheating in written work, and shows an unwillingness to profit by the
mistakes of others.
Actively helps to rid the school of bad influences or environment.

Citizenship
The student who demonstrates citizenship:

Understands the importance of civic involvement.


Has a high regard for freedom, justice, and respect of the American form
of government.
Demonstrates mature participation and responsibility through involvement
with such activities as scouting, community organizations, and school
clubs.

Candidates receiving a majority vote of the faculty council will be inducted into
the National Junior Honor Society.
Once inducted into the NJHS, students will be expected to maintain a grade point
average of a 3.2 or above to continue as a NJHS member.
