Long Nursing Care Plan for S.V.
Purpose of the Assignment
The purpose of the assignment is to enable the student to create a care plan for the
assigned clinical patient, to include nursing interventions, patient education on
medications and disease processes.
Approach to the Assignment
This assignment was a detailed care plan based on a patient I had during a clinical
rotation in my medical-surgical course. This assignment involved the creation of five
pathophysiology sheets directly related to my patient and eight nursing intervention with
the associated planning and outcomes.
Reason for Inclusion
This assignment illustrated my ability to plan patient specific education and healthcare
planning.

Critical Thinking
Revises action plans and goals based on evidence rather than conjecture.
My care plan was based on evidence-based practice and was created specifically for my
patient.
Research
Applies research-based knowledge from nursing as the basis for culturally sensitive
practice.
The nursing interventions used in the care plan were according to NANDA guidelines but

addressed my patients own unique needs.

Leadership
Assumes a leadership role within ones own scope of practice as a designer, manager,
and coordinator of health care to meet the special needs of vulnerable populations in a
variety of practice settings.
Although I had a clinical instructor, I was responsible for my patients care, daily goal,
education and health promotion.
