Student Name: Justin Jones
Age: 12
Grade: 6
Instructional Reader Type: Intermidiate
Independent reading level: 5th
Instructional reading level: 6th
Frustrational reading level: UM
Notes on reading:
Justin overall is a successful reading, as he is a 6th grader reading and comprehending at a 6th grade level.
However, he definitely has room for improvement! I want to focus on comprehension and building
meaning from why or what we have just read. He needs a lot of instruction and practice with writing. I
think he will benefit from scaffolding with prewriting and working through that together. As he reads, he
reads very quickly, so I think slowing his pace would be beneficial. We are also going to practice phrasing
and using expression while reading.
