WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.

What are the schools injury policies? (e.g. What should you do if a child gets
injured?)
o Depending on the severity of the injury, Cleveland Elementarys After
School Program will be contacted and proceed.
What documents must you complete?
o If an accident or injury were to occur an incident record would be
filed. In the event of a head injury/ concussion, a Concussion record
would be filed. Both of these documents would be filed by the Staff of
Cleveland Elementarys After School Program.
Where is the first aid kit?
o The first aid kit is located inside the Gymnasium inside a closet next to
the entrance.
Who should you contact in the event of an injury?
o First, a Staff member of Cleveland Elementarys After School Program
should be contacted. Next, the family of the injured individual will be
contacted. Depending on the case of the injury, if necessary, 911 will
be contacted before the contacting of the family.

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures?

In the event of a fire, if the students are located outside on the blacktop
or grass area they must line up and walk to the open grass area. If the
students are located inside the Gymnasium, the students must line up
and walk to the front of the school building. A head count will be taken
then reported to the Staff members of Cleveland Elementarys After
School Program once everyone has reached the designated area.
Where are the fire exits?
o Inside the gymnasium, there are four exits: two on the far left side of
the building leading to the playground, another facing the front of the
school, and another facing the Staff parking lot, opposite of the two
exit.
If outside, there are no exits only walking calmly to the open
grass area.
Where is the meeting point?
o The meeting points are either in the open grass area, if the students are
already outside. If the students are inside the gymnasium, the meeting
place is in the front of the school.

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children?
o In the event of an unauthorized individual attempting to gain access of
a student, we must redirect them to the gymnasium to speak to the
Staff members of Cleveland Elementarys After School Program.

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted?


o In the event of a school lockdown, students who are outside must go
into an open building and remain quiet until the clear. If students are in
the open grass area they may enter into classroom 29. If they are too
far to reach the classroom, they must remain in the open grass area and
lay flat on the ground. If the students are inside the building they must
remain quiet and hide.

ABOUT THE ACTIVITY SPACE:


A. Risk assessment of activity space: Identify all potential hazards in the
OUTDOOR activity space (draw a map of the area and indicated all potential
hazards).

B. Risk assessment of activity space: Identify all potential hazards in the


INDOOR activity space (draw a map of the area and indicated all potential
hazards).
C. Where are the closest restrooms and water fountains?
If inside the gym, there are no water fountains but there is a restroom by the
stage. IF outside on the blacktop there are two water fountains and one restroom. One
of the water fountains and the restroom is closest to the gym. The second water
fountain is located at the further point of the blacktop by the portables.

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs.
- Be aware of student profile/medication and identify school medical
personnel responsible for these students.

PRE-PROGRAM PREPARATION:
A. Where is the equipment located and how will I access it?
The equipment onsite is located in a shed closest to the gym and can be
accessed by a key that is inside with the After School Programs binders and other
keys.
B. Where will I go if it rains?
In the case of a rainy day, the lesson will be taught inside the gym if there is
space available. If there is no space available, the lesson will be done in the hallways
under the roofs.
C. How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)?
Depending on the weather:
- extreme sun or heat: the students will have their lesson near a shady
area and have multiple water breaks.
- rain or thunderstorm: the students will be shielded away from the
rain or thunderstorm by either going inside the gym or being under some sort of roof.
- extreme winds: the students will have their lesson on the side where
the school buildings will shield and protect them or the lesson will be done inside the
gym.
D. What will I do if a student is not appropriately dressed for Physical
Education?
If a student is inadequately dressed for physical activity their participation will
be adapted to the amount of movement they can do or be given a task on the side.

E. Where can I leave my personal belongings?


I can either keep my personal belongings near my teaching area or in the trunk
of my vehicle.
F. What access, if any, do I have to the teachers lounge/facilities?
If I require access to the teachers lounge/facilities I can go into the office at
the front of the school and simply ask the office help to use the facilities.
Additional Notes:
In the case of an earthquake, students are to stay away from all trees and
buildings that could potentially fall on them.
In the case of a school wide Lockdown, students and staff are to run into the
nearest building or restroom and remain quiet until notified. If the students
and staff members are too far from a building they must lay on the ground in
the grass, remain quiet, and not move until notified.
