Objective: to increase student awareness of his/her learning progress by independently


demonstrating critical thinking, problem-solving, and decision-making skills. The purpose of
self-evaluation is to enhance clinical skill development.
Assignment: the student will submit weekly written Self-Assessments by the end of each week
and adddress the following:
1. Describe what you feel you did well in clinic today/this week:

Monday: White Bird


o Patient was a 2.5 sub, 8 months pregnant. Patient had difficulty getting numb,
Angie, RDH helped administer more anesthesia. Initial was a Right I.A.
Angie administered a higher IA using Lidocaine 2% w/ epi 1:100,000.
Patient had to sit mostly upright for comfort.
Tuesday Afternoon: Started a 2.5 sub patient 4 BWS, Perio Assessment,
Microscope, Dental Exam, cleaned one quadrant.
Tuesday Late Evening: Started a 2 sub patient 4 BWS Vertical, Perio Assessment,
Microscope, Dental Exam, Patient was tired and chose to leave early so I was
unable to start cleaning.
Wednesday: 4J Pedo Clinic. Child was 8 years old, very well behaved. 4 sealants
placed. Cleaning and x-rays went well.
Friday AM: Special Care Client Congenital Hearing Loss.
o This appointment started a little rough. The translator was unable to make it.
AT 8:15am I contacted the translator because neither she or my patient were
there. She told me she could not make it, and that my patient had shown up
and our clinic was closed. She was able to contact him and told him to wait at
a nearby restaurant and I drove and met him. He followed me back to the
clinic. We were able to communicate through simple sign language such as
thank you, more, etc. We also used a pen and paper to write out process
of care and what would be happening next. I also had visual aids for home
care procedures such as sulcular brushing and c-shape floss technique.
Overall a great experience, and he was very nice to work with.
Friday PM: Community Dental Health.
o Patient was a 2.5 sub and only had 11 teeth. 2 partial oral prosthesis. Heavy
plaque, and very dense calculus. Patient had recently finished treatment for
throat cancer, so we did not take any x-rays. Patient had a 5x8mm sore on the
left lateral side of tongue he is going to come back this next Friday for Dr.

Abbott to assess. Intraoral photos were taken of his swollen neck from throat
cancer and side of tongue.

2. Describe what did not go well in clinic:

The only thing that did not go well was my patient at white bird not being able to
be anesthetized and making the cleaning difficult because she was so sensitive.

3. How can I plan for improvement to meet these challenges the next time they present?

Even when Angie administered anesthesia, patient never was fully numb. Maybe
next time I could have used either topical benzocaine or oraqix in addition to
anesthesia.

4. My clinical instrumentation and patient treatment goal(s) for the next clinic session is/are:

Monday: Recall Appointment for my Case Study Patient.


Tuesday Afternoon: RCA
Tuesday Evening: Administering Nitrous Oxide. This patient was Careys
patient, and only has one quadrant left to be cleaned.
Friday AM: CRC patient need to complete radiographs. Perio assessment was
completed this last Saturday.
Friday PM: Senior Day
