Objective: to increase student awareness of his/her learning progress by independently


demonstrating critical thinking, problem-solving, and decision-making skills. The purpose of
self-evaluation is to enhance clinical skill development.
Assignment: the student will submit weekly written Self-Assessments by the end of each week
and adddress the following:
1. Describe what you feel you did well in clinic today/this week:

Monday: NPI
o This first patient was an exchange student from Saudi Arabia, he spoke
English very well. His chief concern is pain his 3rd molars are starting to
erupt. 2 sub. I gave this patient away for someone to use as a caries
management. This patient had several areas of decay.
o Second npi patient was a 2.5 sub patient. I am not sure I will be start a new
patient, I may be able to clean his teeth during my independent study time on
Wednesdays or Saturdays.
Tuesday PM: I met my patient in the lobby to bring him back and he said he was not
sure he wanted to do the appointment today. He said he needed to be somewhere at
2:30pm, even though we had discussed that morning the appointment time was from
1 4:30pm. This patient has bipolar disorder. The reason I state this is because the
first two times I saw him, he was very friendly and warm, this time however, he
seemed very upset with me. He told me that at the last appointment, I was an hour
late coming to get him from the lobby, also at the previous appointment, he left early.
When I mentioned this, he said he didnt recall that happening. Then he said his neck
hurt badly from the last appointment being so long, and that real-world
appointments are 45 minutes, and we shouldnt be taking this long. After I quickly
explained about this being a college clinic, he said he was canceling his appointment
and was not sure he wanted to continue treatment with us. I have not heard back from
him since.
Continue Tuesday PM: Joe, the denturist, had someone at his chair. Sharon heard
that my patient canceled and walked this patient over to my chair. At first I was
happy, because he had a full upper denture only to find out it is new and needs a
few more relines before it can be cleaned. This patient only has teeth on the mandible
and is severe perio case type IV. Previous dental exam has suggested all teeth be
extracted. Furcation involvement, mobility, and 8-12mm pocket depths were found
during the perio assessment. His sub calculus is 3.5 4 sub. I was only able work on

3 teeth. The calculus is difficult to access because of the deep pockets. I will be
seeing this patient on an independent study clinic.
Tuesday Evening: Initial appointment on a patient I recruited. He could not
remember the last time he had a cleaning. He drinks a lot of soda, and uses chewing
tobacco. 2.5 3 sub. He has two areas that qualify for my CRC. I was not able to
start the cleaning because he had to leave early for a soccer game.

2. Describe what did not go well in clinic:

I was hoping to get a bit further on the patient Sharon gave me. However, this
patient made me write a bunch of notes in his chart about his experience with
extractions from clock tower and then Dr. Dunham talked with this patient for
about 45 minutes about his current condition and the chances of retaining
mandibular teeth and time frame.

3. How can I plan for improvement to meet these challenges the next time they present?

This is an interesting situation. How does a hygienist go about asking the Dentist
to shorten a conversation? Not sure. I also wanted to document as the patient
requested. Everything took a long time, which really cut into my treatment time.
