Assessment Protocol

Preparation
1. Record Review: TRS is required to review clients record prior to administering the standardized
test and interview.
2. Standardized Test: World Health Organization Quality of Life-BREF [WHOQOL-BREF]
(World Health Organization, 2004). TRS will review administration instructions. Client will be in
a quiet, private computer lab with minimal distraction and proper lighting. They will be seated at
a computer where the test will be administered. TRS will always be present during standardized
testing to answer any questions or concerns.
3. Interview: Client will be seated in a quiet, private, and comfortable location with minimal
distraction. The TRS will be positioned at a comfortable angle in relation to client (not directly in
front of client and not directly adjacent to them). TRS will be prepared with Copper Hills Youth
Center standardized interview questions (see attached), a writing utensil, and any specific notes
from the record review or standardized test related to the client.
4. Observation: TRS will be trained in using the electronic Copper Hills Youth Center (CHYC)
agency specific observation checklist. This will be used throughout treatment of the client.
Administration
1. Record Review: TRS will review clients record prior to administration of standardized test and
interview. Notes will be made of details that may be important to address during the interview.
2. Standardized Test: TRS will find client and introduce themselves. Therapist will escort client to
the computer lab. Client will be given the WHO Quality of Life Scale. See attached WHOQOLBREF for administering instructions. TRS will be present at all times during the test. If at any
time, client becomes agitated or upset, take a break and return to the test at a later time. Scripting
for the introduction will be as follows:

Therapist Introduction: My name is ________. I am the Recreational Therapist and I


am part of your treatment team. In Recreational Therapy we use recreation as a tool to
help you reach your treatment goals. This test will help us determine what your treatment
goals might be. Do you have any questions?

WHOQOL-BREF Administering Instructions: This questionnaire asks how you feel


about your quality of life, health, or other areas of your life. Please answer all the
questions. If you are unsure about which response to give to a question, please choose the
one that appears most appropriate. This can often be your first response. Please keep in
mind your standards, hopes, pleasures and concerns. We ask that you think about your
life in the last two weeks(continue with attached instructions.)

3. Interview: Will be administered following the record review and standardized test. Interview will
take place in a one-on-one setting with a TRS. It will consist of an introduction, body (see
attached standardized interview questions), and closing. If at any time client becomes agitated,
upset, take a break and return to the interview at a later time. Scripting for the introduction will be
as follows:

Hello ________. You might remember me, I am ________. Im the Recreational


Therapist here and I am part of your treatment team. The questions I ask in this interview

will be used to develop your treatment goals which will also be discussed with you before
they are finalized. I brought paper and pencil to make note of your answers so I can
remember them for later use. Do you have any questions for me before we get started?
4. Observation: TRS will report behaviors using electronic CHYC observation checklist after each
RT activity.
Scoring
1. Standardized Test: TRS will be trained on how to score the WHOQOL-BREF. TRS must score
according to testing manual. See scoring instructions on attached WHOQOL-BREF.
2. Observation: Use scoring rubric on electronic CHYC observation checklist.
Analysis and Interpretation
1. Record Review: TRS will use background information obtained in record review to develop
interview questions. The record review should be used in conjunction with the other assessment
techniques to develop individualized treatment interventions.
2. Standardized Test: Scores will indicate which aspects of life the client feels are strengths as well
as weaknesses. The test is meant to assess the over quality of the clients life including the 4
domains, physical health, psychological, social relationships, and environment. Based on these
scores, along with the other assessment information, TRS will determine which recreation therapy
programs will be most beneficial to the client. TRS will utilize client strengths and weaknesses to
develop meaningful treatment interventions.
3. Interview: TRS will use the interview to either reinforce or deny information obtained in the
previous assessment techniques. TRS will assess client answers and body language to develop
treatment goals.
4. Observation: TRS will track client progress to help determine what effect the treatment
interventions are having on the client. The checklist will give the client a score based on their
behaviors. Client must score level 3 or above to participate in most RT programs.
Reporting
TRS will compile a client assessment summary based on the record review, standardized test, and
interview. Summary will be completed and reported to the treatment team, as well as entered into clients
electronic chart within 72 hours of admission.
