Background:
I have been XXXXXXXs supervisor throughout 2013 2014 school year. We had met frequently
throughout the school year to discuss concerns regarding XXXXXXXs performance of duties in
both the role of athletic director and as an instructional leader. Beginning in September
documentation was collected, shared with XXXXXX and Dr. XXXXXXXX, and reviewed to work
towards improving XXXXXXXs performance. This documentation continued throughout the
2013-2014 school year and was added to the comprehensive/summative evaluation completed
for XXXXXXX that school year. The issues range from participation and professionalism at
district professional development, communication with all stake holders, and organization
leading to attention to responsibilities. On June tenth I met with Dr. XXXX and reviewed the
evidence and evaluation materials submitted for XXXXX. At that time it was decided that XXXXX
needed to improve his skill set and meet the Iowa Leadership Standards in order to be an
effective instructional leader with the Waterloo Community School District. On July fifteenth I
met with Ms. XXXXXX to review the evaluation and plan for XXXXXXX and it was determined that
XXXXXXX was not satisfactorily meeting standards one, three, and four.
XXXXXXX will need to greatly increase his effectiveness as a building leader. He has displayed
the ability to be successful but his inconsistencies in the areas of punctuality, organization,
communication, and professionalism make it difficult for XXXXXXX to effectively lead staff.
Building administration and Educational Services Administration have expressed concerns with
XXXXXXX being inattentive during administrative trainings and meetings. XXXXXXX has been
observed on his phone, I-pad, or generally off task and this is a documented concern.
XXXXXX must consistently enforce the rules and expectations of the building and district. At
times he is unaware of student misbehavior, ignores the behavior, or he attempts to correct the
behaviors in a manner that neither support the staff or the students growth. XXXXXXXX, at
times, seems to be working to have the students like him or fit in rather than be a role model and
helpful adult.
XXXXXXX is in need of assistance in the areas of organization and communication. In the area of
written communication XXXXXX often produces work with grammatical errors as well as typos.
There have also been several incidents where parents have requested communication and
XXXXXXXX has not gotten back to them in a timely manner. There are times when students
have been suspended but no referral has been written or student schedules have changed and
no communication has gone to the teacher or student.
There have also been times when athletic events were unorganized or did not start on time due
to a lack of communication. Coaches have coached entire seasons without being paid due to
XXXXXXXs lack of organization and communication.
At this time I drafted an intensive plan of assistance that outlined areas in need of improvement,
success criteria, and progress monitoring. It was determined that XXXXXX had deficits in the
areas of communication, organization, and professionalism, and that there needed to be a formal
plan of assistance in order to further monitor XXXXXXXs progress in these areas and to make
sure that these issues were actively being improved. A meeting with XXXXXXX, XXXXXX, XXXX,
and I took place on August first at Middle School in order to review the summative evaluation as
well as the initial plan of assistance. The discussion concluded with an agreement to reformat
the plan into this document and the evaluation was signed and dated.

1. XXXXXX is talented with the use of technology.
2. XXXXXX is able to work with office software such as Excel and use these tools to help
teachers.
3. XXXXXX often if not always volunteers to take on responsibilities.

XXXXXX will need to create a check list for each sport that includes coaches, confirmation
of their credentials, confirmation of their contracts, equipment inventory, and facilities
evaluation for locker rooms and practice areas
XXXXXX will need to create a check list for each athletic event that includes officials
(names, numbers, contacted yes or no), staff working, pay sheets, cash box, rosters, and
any other items or equipment required
XXXXXX will need to create a daily communication log that documents all of his
conversations with stake holders whether face to face, telephone, electronic, or other in
order to ensure he follows through and communicates on all he does
XXXXXX will need to schedule, create, and deliver professional development starting with
the safety work and expanding throughout the year
a. Lesson plans submitted the Monday before delivery
XXXXXX will need to read and review selected sections of What Great Principals Do
Differently by Todd Whitaker. Dr. Wiggins will read the assigned sections and then create a
summary of what he has read and how he will apply the information to his current
practice. Beginning with chapter 19
a. Chapters 1, 3, 5, 6, 11, 12, 13,14, 17, 19
b. One chapter each week beginning August 11 th
Supports:
1. XXXXXX will get constant feedback from building and district administration
2. Once a month building administration and district administration will meet with XXXXXX to
review his progress and evaluate his evidence of success
3. XXXXXX will also work to meet weekly (Friday) with his current building administration to
touch base and progress monitor. Though this will not always be possible it will be a goal.
